RN Home Health Care

We are seeking a Registered Nurse - Home Health Full Time & Part Time/ Per Diem to join our team! You will be responsible for the assessment, diagnosis, and treatment of assigned patients in a home health setup. Let us know which areas you can service and we'll match you with the right referrals!

Responsibilities

• Administration of prescribed medication, treatments, and therapies.
• Patient assessments and coordination of care Health, promotion,
• Providing nurse visits to home health patients. (home visits)
• Administer nursing care to ill, injured, or disabled patients
• Diagnose and establish patient treatment plans
• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating physicians or specialists regarding patient care
• Educate patients about health maintenance and disease prevention

Qualifications

• Previous experience in nursing or home health
• Ability to build rapport with patients
• Strong problem solving and critical thinking skills
• Ability to thrive in a fast-paced environment

• Professional License
• Academic Diploma
• Updated CV/Resume with Experience
• Driver's License
• Auto Insurance
• Active BLS or CPR Certification
• Recent Physical Examination OR Fit-to-Work Note from Physician
• Valid (1 year) TB/PPD Test OR Valid (3 years) Chest Xray
• Covid Vaccine Card/Records (Includes Booster/s)
• Influenza Vaccine (or Declination)
• MMR Immunization Records
• Hepatitis B Immunization Records
• Professional Liability Insurance
• Photo (For Issuing of L&G Nursing Group ID)
• Reference Check (at least 2 credible referees)
