We are currently seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) with admissions experience to join our compassionate, team-oriented environment.

Job Summary:

• Greet and assist residents, families, and referral sources in person or over the phone.
• Review and accept new resident referrals for admission into the facility.
• Collect and verify patient information, including insurance and clinical details.
• Coordinate admission logistics and collaborate with interdisciplinary team members.
• Maintain accurate, up-to-date records using MatrixCare or similar EMR systems.
• Answer inquiries and address concerns promptly and professionally.
• Support smooth transitions from hospitals and other care settings.
• Provide administrative support as needed to ensure an efficient workflow.
• Follow HIPAA and facility guidelines to maintain confidentiality and compliance.

Schedule:

• Monday through Friday
• Includes weekend remote admission coverage
• On-call every fourth weekend as part of nursing administration team

Requirements:

• Graduate of an accredited School of Nursing (RN, BSN, or LPN)
• Current, active RN or LPN license in good standing
• Minimum 1 year of clinical experience in a long-term care or skilled nursing setting
• Strong clinical assessment, communication, and organizational skills
• Admissions or case management experience preferred

Pay: From $42.00 per hour
