What You’ll Do as RN MDS Coordinator:

• Complete and submit MDS assessments (MDS 3.0) accurately and on time
• Coordinate interdisciplinary input for care plans
• Ensure compliance with federal and state regulations
• Optimize reimbursement under PDPM through accurate coding
• Audit clinical documentation and educate staff as needed
• Monitor quality measures and support performance improvement

What We’re Looking For:

• Current RN license in NJ
• MDS experience
• Strong clinical, communication, and teamwork skills
• A caring, empathetic approach to patient care
