The following job functions have been determined as essential to the position of Unit Coordinator at Pleasant View Home.
- Create and maintain an atmosphere of warmth, personal interest and positive emphasis in the assigned neighborhoods.

Maintain CONFIDENTIALITY of all pertinent resident care information to assure resident rights are protected.

Coordinate nursing services with other departments.

Assure that a stock level of medications, medical supplies, equipment, etc., are maintained at adequate levels at all times. Requisition equipment and supply needs as required.

Notify the resident’s attending/alternate physician, as well as the resident’s legal guardian/representative, when the resident becomes critically ill, injured, etc., as outlined within our established policies.
-
Work with the facility’s consultants when necessary and implement recommended changes as directed by the Director of Nursing.
-
Review complaints and grievances and make necessary oral/written reports to the Director of Nursing.
-
In cases of death, may call the funeral home as requested by the family, or as otherwise outlined in our established policies. Assure that the body is properly prepared/cleaned for pick up.

Evaluate emergencies and determine emergency measures to be used, including the use of chemical or mechanical restraints. Notify the physician and record and follow orders.

Make independent decisions regarding nursing care.

Make written and oral reports/recommendations to the Director of Nursing concerning the operation of the nursing unit/shift
-
Assure that the unit area is maintained in a clean and safe manner for resident comfort and convenience by assuring that necessary equipment and supplies are maintained to perform such duties/services.

Assist in standardizing the methods in which work will be accomplished.
-
Notify physician of a resident’s accident/incident. Fill out and complete accident/incident report forms on all such occurrences and chart such information in the resident’s medical record as outlined in our established policies and procedures.
- Assist the Charge Nurse and other nursing personnel in performing nursing procedures as necessary.
-
Attend and participate in your professional association’s activities and programs, etc., to assure that you keep abreast of current regulations and guidelines, as well as professional standards, and make recommendations on changes in policies and procedures to the Director of Nursing.

Assure that nursing personnel, including personnel from staffing agencies, follow our established nursing procedures.
-
Encourage attending physician(s) to review treatment plans, record and sign physician’s orders, progress notes, etc.
-
Give/receive nursing report at beginning and end of shift duty.
-
Check residents daily to assure that prescribed treatment is being properly administered by nursing personnel. Evaluate residents’ physical and emotional status. Record findings in the residents’ charts.
-
Prepare/administer medications as ordered by the physician. (NOTE: Medications set up by one nurse MAY NOT be administered by another nurse).
-
Answer telephone inquiries as outlined in our established policies and procedures.
-
Chart nurse’s notes in an informative and descriptive manner that reflects the care provided as well as the resident’s response to the care.
-
Complete required record keeping forms/charts upon the resident’s admission, discharge, transfer, etc. File in the resident’s chart and/or forward to appropriate department.
-
Receive verbal orders from attending/alternate physician and chart on the physician’s order sheet.
-
Assist in orientation of newly assigned personnel as well as supervise and instruct other nursing personnel.
-
Review resident care plan, treatment plan, etc., at least monthly, and make recommendations to the Director of Nursing.
-
Order prescribed medications, supplies, and equipment as established by our policies and procedures.
-
Write resident charge slips and submit to the designated facility personnel.
-
Maintain the daily census report and submit to the designated facility personnel.
-
Review physician’s order, transfer/pre-admission, etc., for completeness.
-
Orient new resident/family members, as requested.
-
Forward new diet orders, and/or diet changes to the dietary department.
-
Transcribe physician’s orders to resident charts, medication record, treatment plans, etc., to assure that they remain current.
-
Verify the identification of the resident before administering medication, treatment, etc.
-
Requisition and arrange for diagnostic and therapeutic services, as ordered by the physician, either within the facility or by outside arrangements, in accordance with established policies and procedures.
-
Assure that nursing personnel are providing adequate nursing care in accordance with established nursing service procedures.
-
Admit, transfer and discharge residents.
- Check resident charts daily for specific treatment, medication orders, schedules, etc.
-
Administer professional service such as; catheterization, tube feeding, suction, applying and changing dressing/bandages, packs, colostomy and drainage bags, taking blood, sputum, urine specimens, massage and exercises, care of the dead/dying, etc., as established by our policies and procedures.

Assure that established infection control and aseptic techniques are followed by all personnel when isolation procedures become necessary.
-
Make rounds with the physician and assist with examinations and treatment.
-
Notify physician of auto-stop orders on specific medications as established by our policies and procedures.
-
Review medication reports for completeness of information, accuracy in the transcription of physician orders, and adherence to stop order policies.
-
Review resident care plans for appropriate resident goals, problems, approaches, etc., to ensure appropriate nursing care is being rendered.
-
Carry out restorative and rehabilitative programs, including self-help/care.
- May perform minor housekeeping duties around the nursing unit.

Provide leadership to nursing personnel assigned to your unit.

Notify the attending physician and supervisor/department director when a resident is discharged without approval. Assure that appropriate documentation is made in accordance with established policies and procedures.

Report all discrepancies of physician’s orders, diet, charting, etc., to the Director of Nursing.
-
Report all hazardous conditions and equipment to the Director of Nursing or to the maintenance department immediately.
-
Check food in resident rooms that is brought in by the resident’s family, or visitors, in accordance with established policies and procedures.

Assume the authority, responsibility, and accountability of directing your shift.

Perform other related duties as assigned.
Education & License Required:
Nursing Degree from an accredited School of Nursing
Active State of Kansas Registered Nurse license
Experience Preferred:
A minimum of one year experience in Geriatrics, and one year in a supervisory position in a health care facility. The Unit Coordinator will receive on-the-job training under close supervision.
