Position Summary

Responsible for ensuring evidence based care is delivered to all patients.


Responsibilities

  • Coordinates the daily operations of the specialized clinical program with a strong focus on quality assurance and improvement

  • Responsible for working in collaboration with the healthcare team to coordinate the care and service to patients across the continuum of care

  • Collaborates the plan of care utilizing standardized care protocols in accordance with nationally recognized evidence based care guidelines

  • Responsible for managing the program including but not limited to community education, prevention, diagnosis, treatment, therapy, and regaining functional status

  • Assists patients in understanding their diagnosis, treatment options and the resources available

  • A wide degree of creativity and latitude is expected

  • May lead and direct the work of others

  • Performs other related duties as assigned

  • Familiar with standard concepts, practices, and procedures within the field

  • Relies on experience and judgment to plan and accomplish goals

  • Regular and predictable attendance is an essential job function

  • Competent to meet age specific needs of the unit assigned

Requirements and Minimum Qualifications

  • BSN required

  • State of Idaho Registered Nurse or licensure pending as documented by temporary licensure OR current RN licensure from a state participating in the "multistate privilege to practice" compact with Idaho required

  • Previous experience in program development

  • Clinical knowledge base and data management skills

  • Knowledge of regulatory standards, evidence-based guidelines

  • Strong organizational and communication skills

Working Conditions

  • Must be able to maintain a sitting and/or standing position

  • Typical equipment used in an office job

  • Repetitive movements
