RN Quality & Clinical Coordinator (Full-Time)
Reports To: Clinical Director

Position Summary

Visiting Nurse Homecare and Hospice of Carroll County seeks a detail-oriented RN Quality & Clinical Coordinator to ensure clinical excellence and quality across home health and hospice services. This office-based role combines chart audits, telephone triage, provider coordination, and staff mentoring to promote high-quality care and documentation readiness. The ideal candidate is a proactive RN with homecare experience who thrives in a collaborative nonprofit environment.

Key Responsibilities

Quality Assurance & Chart Review

  • Conduct routine clinical chart audits and documentation reviews for home health and hospice records.
  • Monitor adherence to state/federal regulations, and agency policies.
  • Identify documentation gaps and provide staff education/feedback.
  • Support survey readiness and track corrective actions for quality concerns.

Clinical Coordination & Office Support

  • Serve as clinical resource for field nurses, providing guidance on care, documentation, and regulations.
  • Perform telephone triage for patient/caregiver calls and coordinate with providers for orders and updates.
  • Facilitate communication between clinicians, physicians, and patients to ensure timely care coordination.
  • Assist intake/scheduling teams with clinical appropriateness questions.

Documentation Oversight

  • Review patient records for accuracy, timeliness, regulatory compliance, and clinical appropriateness.
  • Monitor completion of assessments, plans of care, visit notes, and certifications.
  • Reinforce documentation best practices through staff education.

Staff Education & Support

  • Mentor nurses on regulatory updates, documentation standards, and clinical best practices.
  • Participate in QAPI activities and support agency policy implementation.

Qualifications

Required

  • Graduate of accredited nursing program; BSN preferred.
  • Current, active RN license in New Hampshire.
  • 3–5 years nursing experience, with home health, hospice, or community care preferred.
  • Strong clinical documentation and regulatory knowledge (CMS CoPs familiarity).
  • Excellent communication, problem-solving, and organizational skills.
  • Ability to manage multiple priorities in a small agency setting.

Preferred

  • Experience with clinical audits, quality assurance, or compliance oversight.
  • Proven ability to educate and mentor clinical staff.
  • Familiarity with KanTime or similar homecare EMR systems.
