Job Summary:
To deliver the highest quality care to the Migrant/Seasonal Farm Worker population and community outreach population. This position is primarily responsible for assisting and providing safe, consistent, and appropriate patient care within his or her scope of practice. Promotes and restores patient’s health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; supervising assigned team members. RN’s role will include but not limited to: the assessment, planning, implementation, and evaluation of patients’ statues, plan of care, and goal achievement
General Responsibilities:
Participate in camp visits and provide follow-up care.
- Will assist in registration of M/SFWS, obtaining health history and assessing current level of health.
- Will triage and treat M/SFWS according to established protocols.
- Will refer for clinic examination by SVMS provider those M/SFW’S unable to be established protocols.
- Will assess M/SFW’S learning needs and level of understanding and initiate appropriate teaching and documentation.
- Will initiate interpretive services when necessary.
- Will provide health screenings as determined by yearly program goals.
- Promotes patient independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; answering questions.
- Gives medication as directed by the provider, vital signs, such as blood pressure, temperature, and weight, basic wound care, injections, and immunizations.
- Entering information into Electronic Medical Record (EMR).
- Scheduling appointments and keeping medical records current.
- Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments.
- Performs effective in-take assessments including a brief patient history, chief complaint, current medications, allergies, and significant previous medical/psychosocial problems.
- Performs nursing procedures and treatments, and medication administration according to SVMS Special populations protocols.
- Demonstrates flexibility, open mindedness, and versatility in adjusting to changing environments and requirements as necessary.
- Demonstrates courteous and cooperative behavior with patients, families, co-workers, subordinates, supervisors, and other department staff.
- Maintains patient confidentiality and privacy at all times according to SVMS policies and procedures; uses discretion when discussing patient matters.
- Acts as role model for ancillary personnel
- Practices within the limits of the Nurse Practice Act and understands legal consequences of nursing action.
Maintain adequate inventories of medications and supplies.
- Will take inventory and report to manager any supplies that need to be ordered.
- Will re-stock nurse bag and medication box prior to camp visits.
To perform other duties as assigned.
Qualifications:
- Must be a currently licensed RN in West Virginia, Virginia, and Maryland.
- Five years nursing experience preferred.
- Migrant Farm Worker and/or Community Health experience preferred.
- Outstanding clinical assessment skills required.
Working Conditions:
- The work of the RN is performed at the housing locations for the MSFW population or other assigned area such as packing shed, etc.
- It requires travel among office sites and housing locations and involves working evening and occasional weekend hours.
