RN/LVN

We are seeking a dedicated and compassionate Registered Nurse / LVN- Admissions to join our Hospice healthcare team. In this role, you will be responsible for conducting thorough patient assessments, coordinating admissions, and ensuring a seamless transition for patients into our care facilities. Your expertise will contribute to the overall quality of care and patient satisfaction.

Duties

• Perform comprehensive patient assessments and evaluations during the admission process.
• Collaborate with interdisciplinary teams to develop individualized care plans.
• Document patient information accurately in electronic health record (EHR) systems, ensuring compliance with HIPAA regulations.
• Conduct triage assessments and prioritize patient needs effectively.
• Administer medications and treatments as prescribed, including IV infusions and injections.
• Provide education and support to patients and families regarding care plans and health management.
• Maintain accurate medical records and documentation of patient care activities.
• Participate in quality improvement initiatives and adhere to NCQA standards.
• Facilitate communication between patients, families, and healthcare providers to ensure continuity of care.

Qualifications

• Current Registered Nurse (RN)/(LVN) license in the state of practice.
• Experience in acute care settings such as hospitals or nursing homes is preferred.
• Knowledge of medical terminology, anatomy, physiology, and critical care practices.
• Experience with specific nursing skills including IV insertion, phlebotomy, suturing, catheterization, and airway management.
• Background in geriatrics, hospice & palliative medicine, or working with individuals with developmental disabilities is a plus.
• Strong assessment skills and ability to manage acute pain effectively.
• Excellent communication skills with the ability to work collaboratively within a team environment.

Job Type: Full-time

Pay: $55.00 - $100.00 per hour
