Position Summary:
We’re seeking a Primary Care Physician (MD/DO) who values meaningful relationships and high-quality care. You’ll lead rounds across assisted living and memory care communities, offering personalized, evidence-based medicine that keeps patients stable and out of the hospital. This role allows you to practice at the top of your license while mentoring advanced practice providers within a supportive physician-led model.

Key Responsibilities:

• Conduct on-site primary care and follow-up visits for senior living residents
• Evaluate complex medical conditions and guide chronic disease management plans
• Partner with PAs, NPs, nurses, and community staff to coordinate comprehensive care
• Provide clinical oversight and leadership for interdisciplinary care teams
• Ensure clear, compassionate communication with patients and families
• Support TCP’s mission of proactive, prevention-based medicine

Qualifications:

• Active, unrestricted Minnesota MD or DO license
• Board-certified or eligible in Internal Medicine, Family Medicine, or Geriatrics
• Experience in post-acute, hospitalist, or senior living settings preferred
• Excellent communication, leadership, and clinical judgment
• Valid driver’s license and auto insurance
