We are seeking a dynamic and compassionate Community Liaison Assistant to serve as a vital bridge between community members and our organization. In this role, you will foster strong relationships, facilitate effective communication, and support community engagement initiatives. Responsibilities

• Build and maintain positive relationships with the County Offices of Assistance.
• Coordinate with consumers who are applying for Medicaid. Collect paperwork and prepare applications.
• Schedule assessments for caseworkers with the Philadelphia Corporation for Aging.
• Schedule assessments for the Managed Care Organizations(MCOs).
• Home visits to pick up paperwork like bank statements, identification cards, etc.

Skills

• Proven experience coordinating with the CAOs, PCA and MCOs.
• Strong organization skills.
• Demonstrated ability to work with others.
• Competence in data collection and follow up for program evaluation and reporting purposes
• .Excellent communication skills.
• Background in public health, social work, home care, case management or related fields is highly desirable.
• Leadership qualities with the ability to motivate others and foster teamwork.
• Excellent administrative skills with attention to detail in record keeping and scheduling.

Work Location: In person

Pay: From $18.00 per hour
