Full-Time | Sales Representative - Outreach Coordinator for In Home Care Agency
Are you passionate about helping seniors and medically vulnerable individuals remain safely at home? Do you have experience in healthcare marketing, sales, hospice, home health, or senior care services?
We are seeking a professional, motivated, and relationship-driven Community Liaison to join our growing in-home care team. This position plays a key role in building strong referral relationships within the healthcare community and helping families access quality in-home care services.

Responsibilities:
- Build and maintain professional relationships with:
- Hospitals
- Skilled nursing facilities
- Assisted living communities
- Hospice agencies
- Rehab facilities
- Medical offices and case managers
- Generate and maintain referral partnerships
- Conduct in-person visits and outreach within the community
- Represent the company professionally at networking events and healthcare meetings
- Educate referral partners on our in-home care services
- Meet and exceed referral and growth goals
- Collaborate closely with office leadership regarding outreach strategies and community needs
- Track marketing activities, referral sources, and follow-up communication
- Identify opportunities for growth within assigned territories
Qualifications:
- Previous experience in healthcare marketing, medical sales, admissions, outreach, or business development preferred
- Background or understanding of:
- Home care
- Home health
- Hospice
- Healthcare coordination
- Senior services
- Proven track record of meeting sales or outreach goals
- Strong communication and relationship-building skills
- Ability to work independently and manage time effectively
- Professional presentation and demeanor
- Reliable transportation and valid driver’s license required
- Experience working with referral sources and healthcare professionals strongly preferred
