Purpose: To ensure timely, accurate, and client-centered scheduling for Klein Older Adults (KOA) Home Care and PALS transportation services by coordinating visits, managing schedule updates, and supporting smooth daily operations for clients, aides, and drivers.

Essential Functions:

· Field incoming calls from clients to schedule, reschedule, or cancel home care visits.

· Field calls from clients to schedule or cancel PALS transportation rides.

· Receive and process schedule updates or changes from home care aides and PALS drivers.

· Check availability of home care staff and drivers to assign appropriate visits or rides.

· Input and maintain accurate client and schedule information using Ankota Caregiver

· Approve daily home care visits and PALS ride logs within Ankota.

· Monitor schedules for accuracy and completeness, following up on discrepancies as needed.

· May administer and process PALS assessments for new clients, ensuring proper documentation and eligibility evaluation.

· Provide occasional evening, weekend, and holiday on-call coverage to address urgent scheduling needs or service interruptions.

· Attend monthly Home Care staff meetings and JFCS agency meetings.

· Support departmental initiatives and continuous improvement efforts.

Supervisory Responsibility: N/A

Travel Requirement: Considerable local travel (Louisville Metro area) required.

Pre-Employment Requirements: Satisfactory completion of a pre-employment background check.

Education, Training and Experience Requirements:

  • Completion of at least 60 college credit hours in a related field or 3–4 years of progressively responsible scheduling or care coordination experience.
  • Prior scheduling, care coordination, or home care experience strongly preferred.
  • Strong customer service and communication skills.
  • Ability to manage multiple priorities and respond calmly to urgent situations.
  • Proficiency with scheduling software (experience with Ankota Caregiver preferred).
  • Strong attention to detail and problem-solving abilities.
  • Ability to maintain confidentiality and comply with HIPAA regulations.

Pre-Employment Requirements: Satisfactory completion of a pre-employment background and drivers’ record check. Negative TB test and drug screen required.

Physical Requirements and Environmental Conditions:

· Tasks involve sitting, standing, walking, kneeling, bending, crouching, reaching and performing other physical requirements commensurate with demands of the position, specifically conducting home visits.

· Sedentary work which involves sitting most of the time.

· Must be able to speak (expressing or exchanging ideas by means of spoken words)

· Must be able to hear (hearing aids acceptable)

· Close visual acuity to perform activities such as viewing a computer terminal, excessive reading, and preparing and analyzing data and figures (corrective devices acceptable).

· The worker is not substantially exposed to adverse environmental conditions (typical office work).

Pay: $42,000.00 - $47,000.00 per year
