School Age Service Coordinator (Case Manager)

Job Overview
We are seeking a dynamic and compassionate School Age Service Coordinator (Case Manager) to coordinate person-centered services and supports for school-age children with development disabilities and their families. In this vital role, you will manage service delivery, facilitate connectedness with local resources, and foster strong relationships with families, community partners, and service providers. Duties

  • Serve as the single point of service coordination for children with developmental disabilities aged 6-15
  • Maintain a caseload that requires establishing and maintaining contact with children and families to identify concerns and priorities to acquire services and supports
  • Lead and facilitate the planning process, including the development, implementation, and evaluation of school age programs and services designed to meet unique and diverse needs of children with developmental disabilities
  • Coordinate case management services by assessing individual needs, creating personalized plans, and ensuring timely access to resources and supports
  • Build and maintain strong partnerships and relationships with families, schools, providers, and community organizations to facilitate holistic support
  • Collaborate with other resources, customers, and partners to maximize service delivery
  • Attend team meetings regularly
  • Maintain detailed documentation of services provided, progress notes, and compliance records in accordance with organizational policies

Qualifications

  • Bachelor's degree in education, psychology, social work, or related field; associate degree with experience may be considered
  • Strong organizational and time management skills, excellent interviewing and documentation skills
  • Proficient computer and technology experience including the use of MO365, web-based applications, and various technology systems
  • Valid driver's license with acceptable driving abstract (no more than five points)
  • Ability to maintain confidentiality and handle sensitive data

Pay: From $19.50 per hour
