</b></p>This position is located at Craven Correctional Institution in Vanceboro, NC, Craven County. The prison is a Medium Custody Adult Male Felon Diagnostic Processing Center which houses 850 offenders. Employees in this class carry a caseload of inmates, which involves supportive counseling, conducting assessments, developing case plans, answering and following up on inmate questions and problems, and making recommendations to the classification committee.

<ul><li>In addition to regular case management duties, each case manager will also assist in major dynamic programs or coordinate major or specialized programs for the rehabilitation of offenders, such as offender work assignments, education, or recreation; substance abuse programs, community volunteers, offender library and special projects involving offender art show/writing contests, volunteer banquets, as well as transition planning and coordinating.
</li><li>Employees in this class plan and coordinate transition (re-entry) prior to parole or release into the community.
</li><li>Case managers may have mental health offenders on their caseload which requires additional time to manage.
</li><li>Case managers are also often required to interact with community organizations in their performance of duties.

Correctional Case Manager

<br>The overall purpose of case management in the Onslow County Detention Center is to assess an offender’s criminogenic needs and connect them to treatment and services, helping them remain substance and crime free once released into the community. The Case Manager will work within the Onslow County Detention Center and conduct assessments for all assigned non-violent adults with substance use or co-occurring mental illness and substance use. High priority will be given to those with moderate to high risk of reoffending to reduce the risk of criminal justice recidivism. The Case Manager will coordinate with the current correctional behavioral health providers along with the medical unit and therapy staff to prepare individualized case plans, including referrals to treatment and services upon release from the Onslow County Detention Center. In addition to recovery-based services, the Case Manager will address housing, employment, education and health and wellbeing. Case management services and support takes place both in the Onslow County Detention Center and in the greater Onslow County community and surrounding counties if indicated. Case management will continue for a minimum of 90 days after release with 6 month and beyond data tracking. </b></p><p>The Forensic Case Manager provides services and identification, screening, treatment planning, case management, and court reporting of eligible participants living with mental illness or co-occurring disorders.</p><p>
</p><p></p><p><b>What are the key responsibilities:
</b></p><ul><li>Act as liaison to the courts and correctional facilities. Provide case-finding and screening services for cases within the Court in order to ascertain appropriate service plans for participants.</li><li>
Follows and implements a comprehensive screening to effectuate referrals to treatment and accompanies and/or arranges transportation for clients to the appropriate facility when necessary.</li><li>
Submit monitoring reports to the MMHC Resource Coordinator in a timely manner prior to each scheduled court appearance describing participants compliance with court approved treatment plans.</li><li>
Immediately notify the court if participants commit major infractions.</li><li>
Maintain case documentation available for review by supervision at any time, i.e. client records, including progress notes and all other relevant information to assist clients and to comply with program requirements.</li><li>
Assist clients in applying for and receiving public and private benefits.</li><li>
Maintain a caseload and monitor client participation in the treatment program through case management appointments, reviewing progress reports and conferencing cases with treatment personnel.</li><li>
Make periodic visits to treatment facilities and maintain accurate log of visits.</li><li>
Prepare and present written and oral status reports to the criminal justice system in a timely manner regarding client's progress.</li><li>
Ensure weekly, bi-weekly, or clinically appropriate level of contact with program participants and treatment providers.</li><li>
Participate in case conferences weekly or as needed.</li><li>
Investigate and arrange supportive services and/or entitlements for clients.</li><li>
Attend and engage in relevant trainings or continuing education opportunities as directed by Program Director.</li><li>
Conduct supervised toxicology on participants as needed.</li><li>

This position is responsible for providing transition case management services to inmates in accordance with the requirements of the contract and the principles of the GEO&rsquo;s Continuum of Care. The key components of the GEO Continuum of Care include specialized staff training, enhanced GEOtrack case management, evidence-based interventions, education and vocational programs, transition planning, and post release support. This position is responsible for working with inmates within one year of release on the development of transition plans and connections to post release community-based resources. Participates in appropriate training for designated responsibilities.</li><li>

Conducts individual and group cognitive behavioral treatment sessions as needed.</li><li>
Establishes a relationship with a statewide network of community-based service providers that address inmates&rsquo; needs and works with released inmates to eliminate barriers to successful reintegration.</li><li>
Actively engages community resources to work with released inmates emphasizing the critical first months of release. Works to connect community resources and pro-social people with inmates prior to release via telephone or through visits at facility.</li><li>
Works to ensure inmates obtain eligible benefits and proper identification prior to release.</li><li>
Assures timely communication and follow up with State personnel in the best interest of the inmate and successful reintegration.</li><li>
Invites community-based providers to participate in resource fairs at the facility.</li><li>
Gains an understanding of public domain services such as National Institute of Corrections, National Re-Entry Resource Center, and FDIC MoneySmart program.</li><li>
Reviews opportunities to transition inmates into their communities through community-based services such as transition centers and community residential centers.</li><li>
Attends scheduled staff meetings, promotes communication and the proper flow of relevant information between admission, staff and inmates.</li><li>
Compiles statistical data as needed for reporting purposes.</li><li>
Communicate effectively and coherently to administration, staff, inmates, visitors, and the general public, particularly in situations requiring tact, diplomacy, understanding, fairness, firmness and good judgment.</li><li>
Maintains appropriate documentation of inmate contacts as detailed in the agency program procedures.</li><li>
Performs other related marginal duties as assigned by management.</li></ul>
Qualifications:

Minimum Requirements</b></div><ul><li>
High school diploma or equivalent certification required. Bachelor's degree in criminal justice, sociology, or a closely related field is desirable.</li><li>
Two (2) years of experience working in a correctional setting required.</li><li>
Possess or develop a thorough understanding of the Principles of Effective Intervention that empirical evidence demonstrates reduces recidivism</li><li>
Must possess the ability to work with a team of professionals and have the ability to implement team strategy approaches to work assignments.</li><li>
Must be flexible, cooperative and empathetic to needs of staff and inmates.</li><li>
Ability to prepare written reports; to communicate effectively, both orally and in writing; and to learn rules, regulations, and security procedures required by the facility.</li><li>
Ability to work with computers and the necessary software typically used by the department.</li></ul><div>
