 The Director of Case Management is a strategic and operational leader responsible for overseeing the full lifecycle of claims from post-signature through approval and payout. This role ensures that all cases are processed accurately, efficiently, and in compliance with state requirements while meeting monthly filing and approval goals.
 
 
 This position combines hands-on claim processing expertise, team leadership, and process optimization, serving as the central authority for claim execution, quality control, and continuous improvement within RAD.
 
 
 This role carries direct accountability for converting signed claims into filed claims and ensuring that monthly Filing to State Fee targets are consistently achieved.

The Director of Case Management is accountable for the organization, sequence of services, and resources that are necessary and appropriate for the achievement of patient outcomes within effective time frames on a specific group of patients. The Manager reports to the Director of the Case Management and Social Work Department and is responsible for providing (24/7) leadership and management of case management, social work, utilization management, and discharge planning. The Manager assists the Director with processes and outcomes of clinical care teams and programs designed to address patients and families' clinical psychosocial and financial needs. Scope of responsibilities includes training, supervision, and evaluation of team members and participation in goal setting, program planning workflow process monitoring regulatory compliance, staff productivity assessment of good customer service quality of work, outcome evaluation, and continuous quality improvement. The Manager represents Henry Mayo Newhall Hospital to internal and external customers, including patients and family physicians, ancillary medical personnel, administration, health plans, and third-party payors.
Licensure and Certification:<ul><li>
Current California RN License</li><li>
Current BLS Provider Care</li><li>
National certification from ANCC, ACMA or Case Management Association of America preferred</li></ul>
Education:<ul><li>
Bachelor&rsquo;s degree in Nursing required</li><li>

Experience:<ul><li>
Experience leading innovative case management programs</li><li>
Experience in utilization management and discharge planning required</li><li>
Leadership experience in acute case management required</li><li>

Comprehensive knowledge of laws, regulations and professional standards affecting case management practice in an integrated delivery system: including but not limited to: CMS, Title 22, CHA Consent Manual, and TJC</li><li>
Knowledge of ACMA, CMSA, and NASW Standards of Practice</li><li>
Excellent verbal and written communication skills</li><li>
Ability to promote teamwork and to effectively function in teams, both as a leader and as a team member</li><li>
Working knowledge of Meditech and, Inter-Qual Criteria</li><li>

<div>POSITION SUMMARY
Under the supervision of the Clinical Director (future Community Health and Wellness Director), the Case Manager partners with clients and the community to improve the wellness and health outcomes for Native Americans that are in our service area. The Case Manager will perform case management activities focused on linking clients to behavioral health care, psychosocial supports, and appropriate community resources. The Case Manager will routinely perform client intakes, service planning, safety planning, data collection, and documentation in our Electronic Health Records system.
ESSENTIAL DUTIES

Complete client intakes and plan case management goals.</li><li>
Proactively identify social supports needed for long-term stability of clients and work with clients to build a pathway to accessing such supports.</li><li>

Assist clients in applying for financial, housing, and other benefits, including completing housing, disability benefits, and employment related applications. Identify and support the client's use of appropriate external resources.</li><li>

Appointment reminders, follow-up, and transportation scheduling.</li><li>

Collect and report Government Performance and Results Act (GPRA) data.</li><li>
Collect site-specific data and assist in preparing monthly and annual service report.</li><li>
Assist in development and implementation of health promotion/disease prevention events.</li><li>
Represents Native American LifeLines of Baltimore at community outreach events, including some evening or weekend events.</li><li>
Participates in weekly case supervision with Clinical Director/Community Health and Wellness Director, and staff and site meetings.</li><li>
Create database of community resources and assist with social marketing.</li></ul>

Strong professional ethics.</li><li>
