Summary</b></p>
<p>
The Healthy Families - Family Support Specialist (HF-FSS) optimizes the family home environment for the physical and mental well-being of children who are at risk for abuse/neglect. The HF-FSS works in the families' homes to improve the quality of parenting by providing support, connecting parents to available resources to strengthen the parent-child relationship by improving parenting and independent living skills.</p><br>

Adheres to the Healthy Families America Critical Elements and Healthy Families Indiana, and Healthy Families LaPorte County policies and procedures;</p><br>

Establishes a Family Goal Plan (FGP) to use as a guide for service for each family that includes goals, objectives, and activities; assist family to meet goals;</p><br>

Teaches parenting skills and model nurturing behaviors during home visits using program approved curricula;</p><br>

Inputs computer data and generates reports in a consistent, accurate and timely manner.</p><br>

Education and/or Experience</p>
<p>
Bachelor's degree in human services (social work, psychology, sociology, family counseling, early childhood, or nursing) from four-year college or university preferred;</p><br>

High school or equivalent diploma required;</p><br>

Experience in working with or providing services to children ages 0-5 and families.</p><br>

Infant mental health endorsement preferred;</p><br>

</b></div><div>The Family Support Specialist (FSS) Assesses, plans, links, advocates, coordinates, and monitors to assist children or young adults still in school in gaining access to needed health and dental services, financial assistance and benefits, housing, education, social services, and other natural supports using a family driven, youth guided planning process.</div>
<div></div><div><b>ESSENTIAL JOB FUNCTIONS:
</b></div><ul><li>Assures that the person-centered planning process takes place and that it results in the individual plan of service, and meets direct and indirect service activity expectations</li>
<li>Assures that the plan of service identifies what services and supports will be provided, based upon medical necessity, who will provide them, and how the FSS will monitor (i.e., interval of face-to-face contacts) the services and supports identified under each goal and objective.</li>
<li>Oversees the implementation of the individual plan of service, including supporting the child and family&rsquo;s dreams, goals, and desires for optimizing independence, promoting recovery; and assisting in the development and maintenance of natural supports.</li>
<li>Assures the participation of the child and family on an ongoing basis in discussions of her plans, goals, and status.</li>
<li>Identifies and addresses gaps in service provision.</li>
<li>Coordinates the child&rsquo;s services and supports with all providers, making referrals, and advocating for the child and family.</li>
<li>Assisting the child and family to access programs that provide financial, medical, and other assistance such as Home Help and Transportation services and Medicaid Enrollment and Spend downs.</li>
<li>Assures coordination with the child&rsquo;s primary and other health care providers to assure continuity of care.</li>
<li>Coordinates and assists the child in crisis intervention and discharge planning, and community supports after hospitalization.</li>
<li>Facilitates the transition process (e.g., from inpatient to community services, school to work, dependent to independent living), including arrangements for follow up services.</li>
<li>Assists the child and family with crisis planning. Participates as member of Children&rsquo;s Crisis Mobile Team.</li>
<li>Identifies the process for afterhours contact.</li>
<li>Monitors and ensures that applicable laws, rules, policies, and procedures are followed.</li>
<li>Reports cases of alleged abuse or neglect to agency Recipient rights Officer and/or with Michigan Department of Health and Human Services Protective Services Workers or Licensing Consultant as prescribed by law.</li>
<li>Provides accurate and timely documentation for the clinical records and data systems, in accordance with Medicaid and agency requirements. Refer to attached documentation grid for NCCMH standards.</li>
<li>May assist in transporting and accompanying individuals and parents to medical appointments, evaluations, etc.</li>
<li>Participates in meetings and other related duties as directed by the supervisor.</li>
<li>Complies with agency requirements for continuing education, peer review, credentialing/privileging, and QIDP and/or QMHP requirements.</li>
<li>May assist, arrange, or provide in-service training programs for consumers, direct support workers, foster care providers, parent, guardians, and interested community members or civic organizations.</li>
<li>Performs other duties as assigned by supervisor.</li>

<p>
The ACFS Family Services Specialist, under the direction of the Child Placement Services Manager, is responsible for overseeing various professional assignments to provide services to socially and/or economically disadvantaged individuals in programs administered by ACFS Child Placement Services. Caseworker-Family Services Specialist carries a caseload consisting of family support services, child and adult protection services, and ICWA Monitoring services.</p>
<p><b>
ESSENTIAL FUNCTIONS: (includes, but is not limited to, the following)</b></p>

<li>Completes program-specific paperwork within established time frames.</li>
<li>Maintains client files according to program and funding guidelines.</li>
<li>Provides 24-hour crisis intervention.</li>
<li>Provides on-call services.</li>
<li>Responds to critical situations involving children and families within defined time frames</li>
<li>Evaluates the safety of living environments through home visits and client interviews.
</li><li>Completes risk assessments with the family to determine individual strengths and needs and makes appropriate recommendations for services based on assessment results.</li>

<li>Reviews the behavioral, medical, and educational needs of each child referred for placement at specified intervals and other times as needed.</li>
<li>Prepares and presents information to the Sault Tribe Child Welfare Committee.</li>
<li>Serves as a Qualified Expert Witness and a Tribal Representative as needed.</li>
<li>Provide specialized knowledge and case consultation per the Indian Child Welfare Act, the Michigan Indian Family Preservation Act, and other various state and federal laws that pertain to child welfare court processes.</li>
<li>Provides in-home supportive services, education, counseling, community resource linkage, referral, advocacy, and other social service interventions to families and children.</li>
<li>Conducts forensic interviews.</li>
<li>Completes a monthly narrative and statistical report.
</li><li>Implement and ensure compliance with various Federal and State Laws and Sault Ste. Marie Tribal Code.

PHYSICAL REQUIREMENTS:</b></p>

Position medium with the lifting of 50 pounds maximum and frequent lifting/carrying up to 25 pounds. Physical factors include constant use of hearing, smell, near/midrange/far vision, depth perception, and field of vision; frequent standing, walking, sitting, bending, typing, and driving in and during inclement weather; occasional carrying/lifting, pushing/pulling, climbing, stooping, kneeling, reaching, manual handling, and use of color vision. Working conditions include frequent exposure to weather, extreme heat/cold, wet/humidity, noise, and air quality. Potential hazards include frequent client contact, infectious exposure, computer use, occasional exposure to moving mechanical parts, electric shock, needles/syringes, and equipment use. Positions provide home-based services, and most work occurs within client homes/communities. At times these environments can be volatile due to drug/alcohol use, criminal behavior, or behavioral health needs.</p>

REQUIREMENTS:</b></p>

Education:</b> Bachelor&rsquo;s Degree in Human Services area: Social Work, Sociology, Psychology, Family Ecology, Consumer/Community Services, Family Studies or Child Development, Guidance/School Counseling Psychology, Criminal Justice, Human Services, or in another major where 25% of the course credits earned toward the degree are in human behavioral sciences required.</p>

Experience: </b>One year of experience working with children and families preferred.</p>
<p><b>
Certification/License:</b> Will be required to undergo and successfully pass a background check. Must maintain a good moral character standing requirements. Must have a valid driver&rsquo;s license and be insurable by the Sault Tribe Insurance Department. Must comply with annual driver&rsquo;s license review and insurability standards with the Sault Tribe Insurance Department. Must complete Forensic Interviewing training and show proficiency in implementing forensic interviewing protocol. Program Overview:</b></p><p></p><p>
Intercept&reg;, developed by Youth Villages, is an evidence-based intensive, in-home program that focuses on keeping families together by preventing out-of-home placements or facilitating reunification after such placements, including foster care, residential treatment facilities, hospitalization, or juvenile detention centers. It is one of the first programs to receive the well-supported designation from Title IV-E Prevention Services Clearinghouse developed in accordance with the Family First Prevention Services Act (FFPSA) having been shown to reduce out-of-home placements and accelerate permanency for children.<br>

This program serves youth from birth to age 18 who are facing serious emotional or behavioral challenges or have experienced trauma, such as abuse or neglect. Services are provided by highly trained Family Intervention Specialists directly in the family&rsquo;s home and community&ndash;Intercept&reg; is not an office-based program.<br>
<br>
Intercept provides intensive, strength-based support tailored to each family&rsquo;s unique needs, ultimately helping them achieve lasting change through evidence-based mental health interventions, along with the development of new parenting and communication skills.</p><p></p><p><b>
Essential Duties and Responsibilities:

Carries a caseload of 4 to 6 families</li><li>
