Job Summary:
The Foster Care Case Manager performs case management services for children and families in regular, specialized or home-of-relative foster care programs. The goal is to emphasize meeting clients&rsquo; environmental and psychosocial needs, as well as implementing a permanent living plan for each child.
We are offering a $1,500 sign-on bonus to licensed candidates who are hired. The new hire must have all DCFS required examinations and certifications completed, including the DCFS Child Welfare Licensure in order to qualify for the bonus.
Essential Duties and Responsibilities:
<ul><li>Conduct monthly home visits or more frequently as the program requires.</li>
<li>Arrange and supervise visits between children and their birth parents.</li>
<li>Transport children as needed for visits with biological parents.</li>
<li>Prepare, maintain and provide documents and reports of cases and appear and/or testify in court as necessary.</li>
<li>Assess birth parents and children for strengths and needs.</li>
<li>Refer clients to services and monitor progress in services.</li>
<li>Complete data entry into client management systems.</li>
<li>Consult and review cases with supervisor and follow the course of action.</li>
<li>May recruit, study and license foster homes.</li>
<li>Work effectively with a multi-disciplinary team to do what is in the best interest of the child/family.</li>

We are looking for:
<ul><li>Bachelor&rsquo;s degree in Social Work, Human Service, Psychology or similar degree from an accredited university.</li>
<li>Passing of all DCFS required examinations and certifications including the DCFS Child Welfare Licensure.</li>

<li>Valid driver&rsquo;s license, access to an insured automobile and acceptable driving record</li>

Northeast Center for Youth &amp; Families, Inc. has a full-time opening for a Foster Care Case Manager for our Malden area office to provide intensive case management to youths. Some duties will include communication and relationship building with client and all collateral partners connected with each client (schools, funding partners, therapists, agencies, foster homes, etc), setting limits for youth, development of interventions, and setting goals toward individual successes in conjunction with client&rsquo;s guardians, therapist and other providers. Principal Duties and Responsibilities
 Case Management<ul><li>
Prepare reports and reviews in a timely, clear, and concise manner per Service Manual guidelines, including individual treatment plans or person-centered plans, contact logs, quarterly reviews, supervision notes, discharge summaries, Child and Family, and other assigned reports, utilizing computer-based clinical management software and other network software</li><li>
Review medical administration records (MARs) and grids at twice a month to ensure timeliness and accuracy.</li><li>
Complete all documentation within agency deadlines, according to regulatory guidelines</li><li>
Participate in the planning and delivery of services regarding admission, treatment, and discharge. This includes coordination and facilitation of Child and Family Team Meetings as well as other services as needed.</li><li>

Supervision responsibilities:

Placement/Intake/Assessment

Crisis Prevention/Management

<li>Participate in weekly rotating &ldquo;on-call&rdquo; schedule to provide on-call advisory and support to children and foster parents,</li>
<li>Document and report any on-call activities and follow-up with team leaders</li>
<li>Report to supervisor or to designated authorities any instances of non-compliance with Necco, regulatory rules/policies, reports of abuse or suspected abuse, neglect, or suicidal ideations - immediately</li>
<li>Ensure all critical incidents are staffed with team leaders and entered into the Electronic Health Record within established timeframes.</li>

Data Entry
<ul>
<li>Complete and Submit Medicaid documentation as appropriate</li>

Team Responsibilities
