GOBHI has an immediate opening for a full-time Regional Child Placement Coordinator (RCPC) and we are flexible on the home office location within the Columbia County, Oregon area.</p><p>
The RCPC is responsible for providing foster children and foster parents with case management, counseling, and foster home support. The RCPC ensures that foster home is a sustainable alternative to residential care.</p><p><b>
Essential duties include:</b></p><ul><li>
Case Management &ndash; utilize Collaborative Problem Solving, conduct ongoing case management and respectfully engage with youth, family, treatment teams, community partners and other GOBHI staff to support the goals of the child and family teams. Complete documentation within timelines and maintain compliance with Oregon Administrative Rules (OAR). Provide 24-hour crisis response at assigned times (in rotation with other team members).</li><li>
Certification &ndash; gather and analyze information and confirm safe, therapeutic, environments through interviews, observations, and other source materials of the proctor foster parents. Conduct home visits, assure compliance, and confirm a safe environment for children in care, re-assess certified families on an ongoing basis and assure recertification compliance requirements are met at a minimum of every year. Assist in the recruitment of new foster homes.</li><li>
Collaboration &ndash; Coordinate services with other agencies to support the child in care and proctor foster parents, partner with child and family team to support the needs of the child in care, travel to community meetings and events to represent GOBHI.</li></ul><i>

What is needed to succeed in this role:</b></p><ul><li>
Experience/Education &ndash; Bachelor&rsquo;s degree required with major study in social work, psychology, sociology, or a closely related field and two year&rsquo;s experience in the care and treatment of children and youth, or Master&rsquo;s degree with major study in social work or closely related field and at least one year experience in the care and treatment of children and youth.</li><li>
Communication skills &ndash; ability to present, recommend, and listen at a high functioning level.</li><li>
Flexible &ndash; able to multi-task in response to time-sensitive and changing situations.</li><li>
Strong planning and organizational skills and follows through.</li><li>
Positive attitude &ndash; ability to approach problems in a positive manner.</li><li>

Foster Care Case Manager (hybrid in Oregon)

<div><h1 class="jobSectionHeader"><b>Summary of Position</b></h1><p>
The primary purpose of the Youth Case Manager is to enhance the quality of the group care resident services and to promote continuity of care through case management for each youth resident of Sacred Heart Center Child Services. This is a licensed group care facility for minors placed by child protection agencies. This position is a program manager and will assist with program development and implementing programming and services for the Sacred Heart Child Services department including casement. This is a non- exempt position.</p><h1 class="jobSectionHeader"><b>
Accountability</b></h1><p>
Reports to the Youth Case Manager</p><h1 class="jobSectionHeader"><b>
Responsibilities</b></h1><ul><li>
Responsible for case management services at Child Services/adolescent group residential program, including ensuring compliance with all regulations and licensing and contract requirements and all program policies and procedures.</li><li>
Shift work when needed to meet licensing requirements for staff to resident ratio.</li><li>
Complete daily records through logging interactions with residents, stakeholder, parents, and others related to the care of the residents.</li><li>
Assist residents in the development of his/her treatment plan and case plan by helping youth residents to articulate goals and provide necessary information in developing and achieving treatment plan goals.</li><li>
Responsible for developing, implementing, monitoring, and updating youth resident&rsquo;s assessments, treatment plans and progress reports.</li><li>
Responsible for coordinating and tracking the delivery of services, treatment referrals, linkage to family, referring agency caseworker and family.</li><li>
Maintain a case management record/file for youth/clients which will include required program and case documentation per contracts; treatment plans assessments, treatment/service plans, progress notes, discharge/termination summaries, etc.</li><li>
Coordinate, facilitate and participate in weekly/bi-weekly team meetings and quarterly treatment team meetings; coordinate cases with treatment team members, placing agencies, counselors, other service providers and attorneys/prosecutors as needed, as well as case plan review meetings with placing agencies, clients, families, and treatment team members.</li><li>
Coordinate with staff and counselors in identifying and arranging educational groups and classes such as independent living skills, anger management, behavior management, etc.</li><li>
Coordinate and assist with preadmission process, orientation, client intakes and assessment of newly admitted youth residents.</li><li>
Responsible for the Cans (Child and Adolescent Needs and Strengths) and ACES (Adverse Childhood Experiences) reporting to the Department of Social Services upon intake and discharge.</li><li>
Facilitate meetings with agencies based on client&rsquo;s needs. (e.g. Schools, Behavioral Health, Job Corps, Independent Living Skills/ Facilities, treatment centers, etc.) And provide advocacy on behalf of each youth resident in securing needed services and resources.</li><li>
Be part of a development team in facilitating and assisting in organizational planning and development of programming and services including assisting needs assessment (program and community), assisting in planning, development and implementation of special projects including family violence, domestic violence and sexual assault programming and services.</li><li>
Assist with the planning and development of outreach resources and activities including prevention programming and activities.</li><li>
Provide aftercare services through the following requirements.</li><li>
Provides other services and work duties as needed to meet the needs of the Sacred Heart Center and its ability to carry out its mission.</li></ul><h1 class="jobSectionHeader"><b>

Qualifications</b></h1><ul><li>
Bachelor&rsquo;s degree from a Regionally Accredited institution in behavioral or social science, or closely related discipline.</li><li>
Master&rsquo;s degree in appropriate discipline is preferred.</li><li>
Three (3) years&rsquo; experience in group care for minors setting or working with abused and/or neglected children, or three (3) years&rsquo; program or case management experience is required.</li><li>
Background in grant writing and reporting.</li><li>
Ability to communicate with and advocate for children and adolescents.</li><li>
Experience working in or with Tribal communities and Native American populations is preferred, experience working on a Reservation is strongly preferred.</li><li>
Maintain confidentiality in all departments.</li><li>
Computer skills, must possess basic computer skills including the use of Microsoft Word and Outlook with the ability to learn job-specific programs and software.</li><li>
Interpersonal skills, must be able to communicate effectively.</li><li>
Possess a valid South Dakota driver&rsquo;s license.</li></ul><h1 class="jobSectionHeader"><b>
Physical Demands</b></h1><ul><li>
Uses both hands and wrists repetitively for prolonged periods.</li><li>
Driving is required.</li><li>
