The Team-Based model of care program named EMPOWR (EMbracing, Progress and Opportunities, We Rise) aims to support transition aged youth (TAY, 16-24) attain mental health support, housing stability, employment and education gains, and community connections. The program provides community-based therapy, case management and care coordination, and peer and family support through a team-based model. The whole team collaborates on one shared caseload, providing youth with an entire team of support, and allowing staff to share in the successes and challenges of their clients.

The Intensive Care Coordinator (ICC) provides support and guidance to family and community members who are a source of strength for the youth. They help bring other providers and the youth's natural supports together. This position is responsible for engaging and assisting foster youth and other clients, family members, foster parents, and other support persons in forming a Child and Family Team (CFT). The CFT works with identified support persons from natural supports (friends, extended family, neighbors, community members) and formal system partners (child welfare, education, juvenile probation, etc.) to comprehensively address the child and family's identified goals.

The position draws on lived experience to provide support and guidance to family and community members who are a source of strength for the youth. They help bring other providers and the youth's natural supports together. Because of their lived experience, they help create a connection with family and other close contacts, and offer a unique perspective to the rest of the team.

<ul><li>Outreach to and engagement with youth; being a strong advocate for the youth within the team</li>
<li>Provide community-based trauma-informed services to foster youth or former foster youth transitioning to or struggling with independent living.</li>

<li>Provide assessments, crisis intervention, stabilization, and safety plans.</li>
<li>Identify and connect youth &amp; young adults with other agencies to address their needs, including housing, medical, mental health, education, employment, advocacy, placement stability, and enrichment activities.</li>
<li>Work collaboratively in partnership with the team to engage and connect clients to appropriate resources and services.</li>
<li>Assist with initial contact and intake, establishing relationships and building trust in order to help clients navigate systems for access to services.</li>
<li>Provide advocacy, targeted case management and community based rehabilitation services to children and their families.</li>
<li>Provide services and participate in agency meetings in a combination of environments including virtual (zoom / phone) and in-person office or community locations (drop-in center, client homes, school, parks, etc.)</li>
<li>Provide stabilization support services to children and TAY in an in-person milieu environment.</li>

<ul><li>Direct, lived experience with foster care, social services, juvenile justice, mental health, residential treatment or group home placements, wraparound services, or other related community resources.</li>
<li>Bachelor's degree in Psychology, Criminal Justice, Social Work or related field (preferred)</li>
<li>Experience providing case management and rehabilitation services.</li>
<li>At least two (2) years experience working with system-involved youth.</li>
<li>Demonstrated ability to work with families who have experienced high levels of trauma and demonstrate high mental health needs.</li>
<li>Ability to work independently and collaboratively as part of a multidisciplinary team.</li>
<li>Proven ability to be organized, manage time well, complete high quality and timely documentation.</li>
<li>Ability to travel by car to meet with clients, generally within a 90 mile radius of the WCC office. Clean CA DMV record, current car insurance and a vehicle; travel is required.</li>

Role Overview
Hale Kipa is looking for a Wraparound Facilitator/Care Coordinator to facilitate and coordinate wrapround meetings and services. <div><div><div>Position Introduction:

Peer Learning Assistants (PLAs) provide academic assistance for students enrolled in a range of target courses, including introductory, upper-division, and instructor-managed sessions. PLAs are responsible for preparing for and staffing weekly small-group tutoring sessions, held in-person and/or online. 
In this role, PLAs help maintain attendance records, facilitate study skills consultations, and communicate with course instructors regarding course materials and session promotion. PLAs also regularly report progress to the Student Learning Center Director and the SLC Lead Team.

PLAs are required to attend the following training sessions at the start of each semester:
<ul>
<li>New Member Onboarding / Basic CRLA Training &mdash; Prior to the start of classes (scheduled based on student availability)</li>
<li>CRLA Training Conference &mdash; Once per month on the last Friday (date TBA)</li>
</ul>
PLAs should expect a minimum average commitment of 4 hours per week, with opportunities for additional hours. Candidates must demonstrate a welcoming and inviting attitude, a commitment to fostering an inclusive environment, and a willingness to perform other related duties as assigned.</div></div><div>

Required Education and Experience:
<ul>
<li>Must be currently enrolled as an undergraduate student in the Fall 2026 Term at the University of Oklahoma.</li>
<li>Hiring contingent upon verification of current student status.</li>
</ul>

<li>Must have completed a minimum of 12 undergraduate hours as an OU Norman Campus student</li>
<li>Must have a minimum 3.0 Cumulative GPA, have earned at least a &quot;B&quot; in the course(s) that will be tutored, and have taken the course at University of Oklahoma.</li>
<li>AP credit and/or professor recommendations can substitute for having taken the course, or in the case of languages being a native speaker would qualify in lieu of having taken the course.</li>
<li>Specific Courses: PHYS 2414, PHYS 2424, PHYS 2514, PHYS 2524, MATH 1473, MATH1503, MATH 1523, MATH 1643, MATH 1723, MATH 1823, MATH 1914, MATH 2423, MATH 2433, MATH 2443, MATH 2924, MATH 2934, MATH 3113, MATH 3333.</li>
