JOB OVERVIEW:

Lead Medical Case Manager oversees care coordination and provides a full range of support for clients diagnosed with HIV/AIDS at the Care Clinic. The Lead Medical Case Manager reports to the HIV Program Manager.

Responsibilities:

<li>Maintain a caseload of people living with HIV/AIDS.</li>
<li>Provide oversight of Medical Case Managers (MCMs) and conduct patient chart reviews.</li>
<li>Conduct MCMs monthly supervisions.</li>
<li>Complete a biopsychosocial assessment and service care goals with each client at the time of intake. Monitor and update service care goals and assessments quarterly in coordination with the interdisciplinary team.</li>
<li>Collaborate with an interdisciplinary team in the evaluation of patient needs and treatment progress.</li>
<li>Maintain contact with clients based on program need, client need, and service care goals.</li>
<li>Documentation of adherence to medical appointments and medical care plan, including HIV medication adherence.</li>
<li>Provide trauma-informed therapeutic support in collaboration with an interdisciplinary team, including crises.</li>
<li>Identify appropriate resources and external support systems to address client needs, including advocating and negotiating linkages to resources.</li>
<li>Available to provide home visits and accompany clients to medical appointments and outside treatment facilities as necessary.</li>
<li>Maintain accurate and timely documentation of all client contacts (using D.A.P. format), forms, and applications in EHR software.</li>
<li>Coordinates and documents Psychosocial Support Groups as required by the Division of HIV Health (DHH).</li>
<li>Assist Program Manager with Quality Improvement Projects as directed by DHH.</li>
<li>Perform analysis and reporting of MCM data as required by DHH.</li>
<li>Provide real-time data support to MCM staff.</li>
<li>Support the assessment of impact and outcomes to improve clinical care and meet reporting obligations.</li>
<li>Contribute as a team member and share in the responsibilities required to maintain operations and serve the mission of PHMC. This includes, but is not limited to, obtaining training, participating in events, flexibility in scheduling, covering the commitments of coworkers when they are unable, and other duties as assigned.</li>

Case Management &amp; Resource Navigation
1. Resource Coordination: Connect clients to essential community services such as housing, employment, transportation, and additional treatment options.
2. Case Support: Assist the medical team with treatment or recovery plans that align with the client's identified goals.
3. Medication Assistance: Assist clients with self administration of daily medications as assigned by the Registered Nurse (RN). Check in medications from the pharmacy if RN is not available.
4. Community Outreach: Represent the program in teams and with community partners to identify and engage individuals who may benefit from services.
5. Crisis Management: Identify signs of distress and provide immediate emotional support or intervention as needed.
Medical Secretarial &amp; Administrative Functions
6. Office Operations: Manage office duties including greeting patients, answering multi-line phones, and handling mail or faxes.
7. Schedule Management: Manage a complex medical schedule for prescribers and nurses. Coordinate, set up, and confirm appointments for clients with medical providers and nurses.
8. Records &amp; HIPAA Compliance: Maintain accurate electronic health records (EHR), process referrals, and ensure strict confidentiality of all client information.
9. Billing Support: Verify insurance eligibility, assist with basic medical billing tasks, chart audits to ensure accuracy and compliance with treatment plans.
MINIMUM QUALIFICATIONS
1. EDUCATION AND EXPERIENCE
A. High School Diploma or GED; one to two years of college education in psychology, social work or related field is preferred,

B. Three to six months experience working in a medical office or social service program,

C. Equivalent combination or education and related experience.
2. LICENSES AND/OR CERTIFICATIONS
A. Valid motor vehicle driver's license (equivalent to State of Utah Class D)

A. Certification: Certified Case manager credentials, which involve formal training in ethics and recovery principles.

As a Field Medical Case Manager at ForzaCare, you'll help injured workers navigate their recovery and return to work. You'll act as the central point of coordination, connecting the injured worker, medical providers, employers, and insurance carriers to ensure timely, transparent, and effective care management.
