 Join our growing practice as a Nurse Practitioner (NP) or Physician Assistant (PA) and bring exceptional team-based care directly to patients residing at Assisted Living and Memory Care communities in and around Davenport, FL and will extend to Lakeland and Kissimmee, FL
 
 
 Our unique, robust model of care goes beyond primary care services &mdash; our multidisciplinary care teams collaborate with patients, their families and other healthcare providers to deliver care that is preventative, proactive and tailored to their unique needs.&nbsp;
 
 
 Under our model of care, Bluestone patients experienced
 
 21% fewer ER visits, 36% fewer hospitalizations and 41% fewer hospital readmissions
 
 compared to patients with similar conditions and complexities over the same time period.
 
 
 Position Overview:
 
 <ul>
 <li>
 Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities
 </li>
 <li>
 Full-time providers care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day
 </li>
 <li>
 Manage a panel of 200 patients (approximately 12-16 patients per/day with 14 being the avg.)
 </li>
 <li>
 Ability to design and manage your schedule&nbsp;
 </li>
 <li>
 Develop and implement care plans appropriate to patients and their desires
 </li>
 <li>
 Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers&nbsp;
 </li>
 <li>
 Access to diagnostics such as labs and x-rays as needed
 </li>
 <li>
 Collaborate with community partners within home care, hospice, and other resources as appropriate
 </li>
 </ul>
 
 Qualifications:&nbsp;
 
 <ul>
 <li>
 Graduate of an accredited Nurse Practitioner Education Program or Physician Assistant Education Program
 </li>
 <li>
 Board Certified/Board Prepared through AANP, ANCC, or NCCPA&nbsp;
 </li>
 <li>
 Licensed or ability to obtain licensure through the Minnesota Board of Nursing or Board of Medical Practice
 </li>
 <li>
 Ability to obtain and /or maintain DEA certification for the State of Minnesota
 </li>
 <li>
 Proven ability to communicate effectively with strong verbal skills
 </li>
 <li>
 Excellent interpersonal and customer service skills
 </li>
 <li>
 Must have a valid and unrestricted driver&rsquo;s license and minimum state-required liability auto insurance; local travel is required

The Physician Assistant provides medical care under the general supervision of a physician and is involved in the evaluation, diagnosis, and treatment of patients.
As a Physician Assistant (PA):
<ul><li>Responsible for the comprehensive coordination of medical care for assigned patients.</li>
<li>Completes all required documentation including admission history and physical assessments within 24 hours, medicine/pediatric consultation assessments, and follow-up notes within 48 hours.</li>
<li>Utilizes ICD 10 terminology and concepts for diagnosis and treatment.</li>
<li>Prescribes medication or other interventions within the scope of PA practice.</li>
<li>Operationalizes systems to accept and treat all appropriate patients up to hospital census capacity.</li>

The Physician Assistant (PA) will have the following:
<ul><li>Graduate from an accredited Physician Assistant Program</li>
<li>Master&rsquo;s degree in physician assistant studies</li>
<li>1-5 years of experience preferred</li>
<li>Current Massachusetts Physician Assistant license registration</li>
<li>US DEA and Massachusetts Registration as Controlled Substances Practitioner, Schedules II-VI.</li>
<li>Current CPR certification is required.</li>
<li>Prior experience in a Psychiatry setting is preferred but not required.</li>
<li>Familiarity with electronic medical records and healthcare technology</li>
<li>Self-directed, demonstrate initiative and professionalism, and exercise expert clinical judgment to function independently.</li>
<li>Skills in interpreting and applying standards and regulations applicable to behavioral health inpatient services (e.g. JC, CMS, DMH, DPH)</li>
<li>Time management, problem-solving, crisis management, and team-building capacity; flexibility, creativity, and ability to manage stress.</li>
<li>Ability to work in a fast-paced environment independently and as part of a team.</li>
<li>Excellent verbal and written communication skills.</li>
