Responsibilities:

Performs patient assessments and collaborates with the care team to develop and implement a plan of care.</div></li><li>
Makes referrals to other disciplines as indicated by the patient's identified needs or documents rationale for not doing so.</li><li>
Promotes patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques.</li><li>
Supervises LPNs and HHAs.</li><li>
Completes documentation timely and accurately.</li><li>
Regularly communicate patient progress to the clinical manager and care team.</li><li>
Plans and provides staff education.</li><li>
Performs on-call responsibilities and on-call services to patients/families as assigned.</li><li>
Participates in clinical development and continuing education programs.</li><li>

Qualifications:

<li>Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal</li>
<li>Implement the individualized POC and recommend revisions to the plan as necessary</li>
<li>Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals</li>
<li>Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care</li>
<li>Perform on-call responsibilities and provide on-call service to patients/families as assigned</li>
<li>Timely submission of all documentation</li>

Under general supervision, the RN performs health services directed towards individuals and families in the community. Nursing competencies include strong analytic and assessment skills, identifies, interprets, and enforces public health laws, ordinances, and policies related to specific programs.<br>

Communicates effectively both in writing and orally, participates in groups to address specific health related and social issues. Identifies the role of cultural, social, and behavioral factors in determining the delivery of public health services. Build community partnerships and establish linkages with key stakeholders. Make home visits to children, youth, adults, and their family. Assess health pf the entire family and develop an appropriate plan of care.</p></div><br>

<p></p><div><ul><li>Complete a nursing case management and home visits according to established protocol.</li><li>
Provide holistic family care and health information related to the assessment of medical diagnosis education and collaborative needs and other family-centered resources.</li><li>
Conduct home visit assessments and reinforce teaching around areas of concern identified during assessments. Provide referrals to appropriate agencies for identified needs.</li><li>
Coordinate communication and services with specialty care providers, primary care, other health care providers, and Medicaid health plans to address case management and care coordination needs.</li><li>
Knowledge of City Departments, Health and Human Services agencies, Qualified Health Centers, and Community Based Organizations at the local and state level.</li><li>
Competence in the use Electronic Medical Record (EMR) and other reporting databases. One year experience in hospital or acute care setting as a Registered Nurse (RN).</li><li>
One year experience in home care, community health or related field as a Community Health Nurse.</li><li>
Response during a public health emergency event is required for this position, including duties not normally performed.</li><li>
Provide assistance with other Health Department clinical operations including immunizations, lead, hearing and vision, WIC, and other services as needed.</li><li>
Assesses health care development, family and educational needs of both the Children&rsquo;s Special Health Care Services (CSHCS) clients and their families. Provides nursing intervention and/or referrals as needed.</li><li>
Coordinates care for the involved families, assists families in processing and procedural questions, and provides general assistance for families in the CSCHS program to assure children with special health care needs receive the services they require.</li><li>
Provide Case Management services to clients, based on their needs.</li><li>
Develops Level I Care Coordination Plan of Care and provide Level I Care Coordination services.</li><li>
Provides Level II Care Coordination services or delegates appropriate Care Coordination activities to the CSCHS? representative.</li><li>
Serves as the local program's contact with health professionals regarding clinical issues.</li><li>
Coordinates and links clients and/or families to appropriate community or public agencies to obtain services or assistance to improve or maintain health educational and social functioning and collaborates with physician as needed.</li><li>
Documents services performed, maintains required program records, including statistics, and that confidentiality of client information is maintained.</li><li>
Ensures that all client services are provided in compliance with established standards of professional practice and ethics, CSCHS Guidance Manual and Health Department policies, procedures, and quality standards.</li><li>
