This position is for a RN Case Manager in the Peckham Center for Cancer and Blood Disorders. This open position will be managing the Leukemia and Lymphoma population. The hours are Monday to Friday 0830-1700. We are looking for someone with Pediatric Hematology/Oncology experience and case management experience.

<b>JOB SUMMARY:</b><br>
Peckham Center Case Manager coordinates provision of care across the continuum for pediatric patients with a hematology and/or oncology diagnosis. The Case Manager advocates for the patient and family through triage, education, and navigation of insurance authorization requirements, as appropriate, to the nursing role. The Case Manager identifies, accesses, and allocates resources to meet the goals of care, as identified by the multidisciplinary team, in partnership with the patient and family.

Participate in periodic conferences, with the Home Health Aide supervisor, concerning the Aide's performance.</li><li>
Chart those services rendered to the patient, by the staff nurse, and changes that have been noted, in the patient's condition and/or family and home situation, make revisions in the nursing care plan, as needed, record supervisory visits conducted with the Home Health Aide, evaluate patient care and progress, and close charts of discharged patients.</li><li>
Provide guidance, and supervision, to the LPN and supervises the LPN, per agency policy.</li><li>
Participate in the educational experiences for student nurses.</li><li>
Participate in the planning, operation, and evaluation of the nursing service.</li><li>
Ensure that all OSHA/ACHC standards are in compliance.</li><li>
Work with Discharge Planners to identify, and plan, home infusion therapy referrals.</li><li>
Assess the patient on the initial visit, including an assessment of home safety, adequacy of refrigeration, storage, and cleanliness.</li><li>
Initiate the patient plan of care and make necessary ongoing revisions; provide service, in accordance with reimbursement, ethical, and legal guidelines.</li><li>
Administer venipuncture and handle insertion of intravenous needles and catheters.</li><li>
Prepare infusion solutions, with the addition of medications.</li><li>
Initiate, monitor, and terminate infusion solutions, and additives, and establish flow rates (if not done by the physician).</li><li>
Prepare clinical, and progress, reports and make referrals for other services, as appropriate.</li><li>
Inform the physician, and other appropriate personnel, of changes in the patient's needs.</li><li>
Train, both, the patient and caregiver and assess the ability of patient, or caregiver, to manage when staff is not present.</li></ul><p><b>

Position Summary
<br>The RN Case Manager is responsible for planning and managing the nursing care of an assigned caseload of patients in home or long-term care settings. In collaboration with the interdisciplinary team, the RN Case Manager ensures that patient- and family-directed goals are achieved using a holistic, patient-centered approach. The role includes providing education, facilitating communication, and coordinating care across various sites. Flexibility in work location across care settings is required.

Minimum Qualifications</b></p><ul><li>
A Diploma or associate degree in nursing is required; Bachelor&rsquo;s Degree preferred.</li><li>
A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
</li><li>The Nurse Licensure Compact (NLC) allows a registered nurse (RN) and licensed practical/vocational nurse (LPN/LVN) to have one multistate license in a primary state of residency (the home state) and to practice in other compact states (remote states), while subject to each state&rsquo;s practice laws and discipline.</li><li>
Current CPR required; CHPN preferred.</li><li>
Minimum 1-year related experience for inpatient units (IPU) and 2 years for other nursing positions. Experience in direct care of patients with life limiting illnesses required. Summary:
<p>
The Care Ally, Case Manager is a key member of the interdisciplinary care team (ICT). They use a collaborative process of assessment, planning, implementing, coordinating, monitoring, and evaluating options and services required to meet the members health and social needs. They act as a liaison between our Members, their Responsible Parties and/or Power of Attorneys (RP/POAs), Advance Plan Provider/PCP, and key Align Senior Care stakeholders. The Care Ally, Case Manager reports to the Supervisor of Case Management.</p>
Essential Duties &amp; Responsibilities:

Responsibilities</b></p><ul><li>
Executes on strategies and goals set by the Align Senior Care Board of Directors, the Senior Leadership Team, and Executive Director for managing and improving overall Member experience.</li><li>
Contacts Plan members to conduct a comprehensive health assessment of the individual, develop a plan of care, and participate in the facilities interdisciplinary care team meeting.</li><li>
Serves as health coach to educate the member, the family and/or caregiver, about disease status and treatment, plan benefits, community resources, and resource options</li><li>
Collaborates with members of the interdisciplinary care team and medical director(s) to facilitate appropriate treatment for members</li><li>
Routinely follows up with member as scheduled to assess progress towards goals</li><li>
Communicates with the member and/or caregiver to assist with the development of health goals and identify interventions to achieve these goals</li><li>
Provide patient-centered intervention, such as making and verifying appointments, performing medication and care compliance initiatives.</li><li>
Acts as front-line support with Members and their RP/POAs to ensure the needs of the Member are met. Serves as a connection point among Members, their Communities, their Care Team, and Align Senior Care internal departments.</li><li>
Regularly engages Align Senior Care Members and RP/POAs in-person or by phone to provide education and assistance with utilizing Align Senior Care benefits. Including but not limited to. checking on upcoming specialist appointments, connecting members to supplemental benefits and providers, identifying immediate Member needs, and answering any questions the Member or RP/POA may have.</li><li>
Communicates Member health updates from Care Team to RP/POAs.</li><li>
Coordinates with the Care Team for non-urgent health or clinical questions.</li><li>
Works directly with internal departments to solve Member Grievances, Utilization Management, and Billing related issues.</li><li>
Updates Member and RP/POA contact information such as changes of address, email, or phone numbers.</li><li>
Actively supports Account Manager in identifying and securing contracts with &quot;preferred&quot; Providers.</li><li>
Assists Members, RP/POAs, and Partner Communities with locating in-network providers and scheduling/facilitation of appointments.</li><li>
Assists with (on request of member or APP) coordination of home health and therapy visits, ordering of Durable Medical Equipment, and utilization of supplemental benefits for Members.</li><li>
Monitors and, if needed, facilitates care team meetings with facility team, member, responsible partie(s) and the APP/clinical team.</li><li>
Ensures documentation of care team meetings and transmits to Plan.</li><li>
Monitors care plan updates, facilitates APP and PCP input into care plan, and distributes care plan as needed to care team members.</li><li>
Monitors midnight reports/community census to help identify member transitions to hospital or other care levels.</li></ul>
Qualifications:
<p><b>
Education &amp; Experience</b></p><ul><li>
One (1) year of clinical practice experience in at least one of the following areas: case management, home health, critical care, medical/surgical, discharge planning, concurrent review, or obstetric/neonatal care.</li><li>
Proficiency using basic computer skills in Microsoft Office such as Word, Excel, and Outlook, including the ability to navigate multiple systems and keyboarding.</li><li>
Case management certification preferred.</li></ul><p><b>
Professional Certification Or Licenses</b></p><ul><li>
