Emergency Assistance

Client Assistance

Community

<li>Be knowledgeable of community social services and interpret social services regulations in a clear, accurate and helpful manner to clients</li>

<li>Communicate appropriately with staff, clients, other agencies and the public using the telephone or in face-to-face contact, one-to-one and in group settings</li>

Pathway of Hope

Outreach and Engagement
<ul>
<li>Conduct regular outreach to social services sites and other locations in the community to identify eligible applicants</li>
<li>Engage and build rapport with target population</li>
<li>Conduct screening interviews with potential applicants in accordance with POH eligibility requirements
</li><li>Present potential participants to POH team to include the corps officer, youth pastor, Regional Coordinator, and Divisional POH Program Manager</li>
<li>Provide appropriate referrals for individuals not eligible for POH services</li>
</ul>
Case Management
<ul>
<li>Perform intake that includes required documentation for admission, referral needs, explanation of program and expectations</li>
<li>Conduct a written assessment on all participants within 72 hours of intake including screening for serious personal safety and mental health issues</li>
<li>Develop a case plan with the participant based on the primary goals of housing stability, increased income/financial resources and skills, and self-determination within one week of intake which includes short-term and long-term goals with objectives.

<li>Schedule regular meeting times with participants to develop and review goals and objectives</li>
<li>Conduct home visits as determined by the case plan</li>
<li>Provide information and referral services as needed</li>
<li>Assist participants in making linkages and accessing appropriate community resources</li>
<li>Provide advocacy services as needed, i.e., court involvement, landlord/tenant conflict resolution, etc.</li>
<li>Provide financial assistance in accordance with program policies and procedures</li>
<li>Coordinate case management efforts with all staff and contracted services to meet individual and family needs</li>
<li>Develop a discharge plan with participants addressing permanent housing stability, economic stability, emotional stability and self-determination</li>

<li>Conduct life skills and budgeting classes</li>
<li>Maintain case files for each participant that includes the assessment, case plan, documentation of progress, challenges, outcomes/accomplishments, discharge plan and summary, and documentation of all follow up contact</li>
<li>Maintain comprehensive and detailed case notes on all participants</li>
<li>Complete required documentation of all educational groups
</li><li>Prepare case records for proper storage after participant discharge</li>
<li>Ensure client confidentiality in accordance with established procedures and regulations</li>

HMIS Case Management &amp; Direct Assistance
<ul>
<li>Maintain comprehensive demographic data as required on all participants</li>
<li>Submit monthly summary of service statistics to the Regional Coordinator</li>
<li>Maintain accurate records of financial assistance provided to participants in their file and in the MAACLink Homeless Management Information System and the Salvation Army Information Management System (SIMS)
</li><li>Complete other reports as requested</li>
</ul>
Agency and Community Networking
<ul>
<li>Attend agency and community meetings as requested</li>
<li>Meet with Regional Coordinator on a regular basis (at least monthly) for individual consultation and case management supervision</li>
<li>Participate in monthly cluster meetings led by the Regional Coordinator</li>
<li>Attend supervisory meetings</li>
<li>Maintain working relationships with community agencies to provide comprehensive services for participants and to stay abreast of current trends and resources available</li>
<li>Provide guidance and support to community volunteers working with the program</li>
<li>Represent the agency's purpose, philosophy, and function to the community</li>
<li>Promote the spiritual dimension of Salvation Army social services programs, emphasizing the importance of Christian influence and evangelism in all Salvation Army ministries, including delivery of social services in contemporary settings</li>
</ul>
Quality Assurance
<ul>
<li>Track and report unmet needs of participants and their families</li>
<li>Document and report effectiveness of service delivery using consumer, case management and community resource feedback</li>
<li>Provide ideas for programming specific to educational or process groups needed for participants</li>
<li>Ensure quality of local data and reporting of information that will support national and territorial POH program outcomes</li>
<li>Ensure relevance and consistency of POH by collaborating with the Regional Coordinator and Divisional Program Manager with ongoing development of policies and procedures, goals and objectives, and outcome-based measures related to the POH</li>
<li>Report any POH challenges and work with the corps officer and POH Program Manager to develop an action plan to address program development needs</li>

Pathway of Hope Program Evaluation and Outcomes Measurement
<ul>
<li>Ensure the accuracy of data entry into the MAACLink and SIMS database</li>
<li>Participate in other program and outcome evaluation activities</li>
<li>Assist Regional Coordinator in the annual POH evaluation/outcomes measurement for Corps and summary/review report to the Divisional Program Manager and Divisional Social Services Director</li>
<li>Assist Regional Coordinator in the completion of quarterly random record reviews and annual review of compliance with standards in order to assure for program evaluation/certification requirements</li>

Education and Training
<ul>
<li>Attend training sessions that support the initial implementation phase and ongoing training requirements for POH</li>
<li>Attend in-service training and outside conferences/workshops as requested and approved by the Corps Officers</li>
<li>Participate in POH workshops and other training events at the territorial, divisional or local level, as requested and/or assigned</li>

Minimum Qualifications:
Education: Bachelor's degree in human service area: prefer BSW from an accredited college or university. Associates degree with extensive experience in strength-based case management may substitute for bachelor's degree as determined on a case-by-case basis.

Experience: Minimum of two years of case management experience in a comparable social service program that offer multi-faceted case management interventions designed to address the needs of those who are vulnerable utilizing a strength-based approach.

<div>The Social Justice Coordinator will serve as an assistant to the Social Justice Director, aiding in every way possible in developing and coordinating the Social Justice &amp; Community Service Ministries, helping in the carrying out of his leadership and pastoral duties. Take part in the Monday Morning Ministry, supporting volunteers and coordinators, while also getting to know our guests.

Keep inventory of supplies for the Monday Ministry, ESL &amp; SSL, and Adopt-a-Family programs and place orders when necessary.

Work with volunteers to maintain upkeep of Monday Morning Ministry supply and storage closets (restock, organize, donate unusable items).

Respond to financial/resources assistance calls providing information and referrals for social agencies and services.

Assess emergency financial assistance requests by meeting with individuals who ask for emergency assistance from the parish. Work with Catholic Charities&rsquo; Parish Partners Program. Maintain files and records of assistance.

Respond to the &ldquo;knock on the door&rdquo; when walk-in visitors come to the rectory in person seeking assistance.

Attend Cathedral events/liturgies that staff are required to serve in.

Maintain periodically vigilance in the Cathedral for a safe environment.

Expectations
The Social Justice Coordinator will be proficient in the use of standard office tools: computer, copy machine, fax, email, etc. He/She will demonstrate organizational skills, as well as an ability to relate well with a diversity of people: staff, volunteers, parishioners, and visitors.

He/She will be a person who possesses a general understanding of the social teachings of the Church and is committed to them; shows special concern for the poor, the homeless, immigrants, the vulnerable, and the marginalized.

<div>The University of Tennessee Knoxville, College of Social Work, through the Social Work Office of Research and Public Service (SWORPS) is accepting applications for a Coordinator for the Tennessee Child Support Employment &amp; Parenting Program (TCSEPP). Within SWORPS, the Tennessee Child Support Employment and Parenting Program (TCSEPP) Coordinator builds a strong evidence-informed and trauma-informed practice that assists, supports, and advocates for non-custodial parents involved with the child support system. The Coordinator, based in COFFEE County, TN, assists non-custodial parents with overcoming barriers to employment in order to become stable, obtain employment, pay child support, become self-sufficient, and be an active parent in their child(ren)'s lives. The Coordinator works with non-custodial parents to promote trust and communication between them and the systems with which they interact as well as with, when possible and appropriate, with the children and custodial parents. The Coordinator also weaves together a supportive network of formal and informal community-based resources for non-custodial parents and provides outreach and education to non-custodial parents and the community-at-large.
This is a grant-funded position and is contingent upon the continued funding of the grant.</div>

</li></ul><ul><li>Develop individual action plans for participants and provide one-on-one case management to participants to assist them with job readiness activities and emotional support and encouragement
</li></ul><ul><li>Maintain accurate records of all participant interactions in appropriate places, i.e. TCSES, Apricot, Excel spreadsheet, etc.
</li></ul><ul><li>Monitor participants&rsquo; child support eligibility &amp; payments; program involvement &amp; compliance as well as employment.
</li></ul><ul><li>Develop and maintain partnerships with local agencies to assist participants with barriers to employment
</li></ul><ul><li>Attend community interagency meetings to learn about and share available resources and information
</li></ul><ul><li>Participate in partner meetings
</li></ul><ul><li>Provide regular updates to Child Support staff and attorneys on participant progress
</li></ul><ul><li>Maintain a court presence to foster a positive relationship with the Child Support legal system and to recruit and/or enroll potential participants
</li></ul><ul><li>Participate in job/resource fairs
</li></ul><ul><li>Fiscal tasks including but not limited to One Card reconciliation and uploading &amp; documenting receipts, monthly gift card reconciliation, and travel reports</li></ul></div>
 <div>Required Qualifications
<ul><li>Experience : Two years of customer/client (case management) experience. In lieu of a degree candidates with two additional years of experience will be considered.
</li></ul><ul><li>Education: Bachelor&rsquo;s Degree
</li></ul><ul><li>Must have valid driver&rsquo;s license, automobile at their disposal, and a current automobile liability policy with continuous coverage.
</li></ul>&middot; Knowledge, Skills, Abilities:
Ability to assess participant needs; Ability to develop positive &amp; trusting relationships with participants and offer emotional support; Ability to positively interact with partners including child support, court, attorneys, etc.; Ability to promote the program to potential participants, partners, and employers; Ability to advocate for participants; Ability to work independently and within a team setting; Ability to travel to multiple counties to serve participants ; Ability to be flexible with respect to changing needs of the project; Excellent oral communication skills; Proficiency with Microsoft Office; Ability to work independently and in team settings; Experience with data management systems

We are looking to welcome a self-motivated, empathetic individual who will provide excellent services to seniors living in our communities. Using your knowledge of resources, you will perform service coordinator duties such as resident assessments, problem solving, and crisis intervention. You will use your interpersonal skills to advocate on behalf of the residents and coordinate educational, informative, and stimulating programs for them while maintaining confidentiality. This role is the bridge between residents and essential services that will help allow them to age-in-place with dignity. Multi-tasking and time management skills are essential in this job. Position will split time equally between Luther Terrace in the Memphis (Raleigh) area and at St. Mark Manor in Moscow, TN.<h3 class="jobSectionHeader">
Key Responsibilities</h3><ul><li>
Case Management &amp; Advocacy: Conduct initial and ongoing functional assessments to identify residents' needs. Help them apply for programs like SNAP, Medicaid, and utility assistance.</li><li>
Service Coordination: Establish and maintain relationships with local healthcare providers, home health agencies, and transportation services.</li><li>
Programming &amp; Education: Organize wellness clinics, financial literacy workshops, and social events to combat senior isolation.</li><li>
Crisis Intervention: Provide immediate support and referral services during resident emergencies or transitions (e.g., hospital discharges or move-ins).</li><li>
Compliance &amp; Documentation: Maintain accurate, confidential resident files in accordance with HUD (or relevant agency) guidelines and privacy laws.</li><li>
Mediation: Assist in resolving conflicts between residents or between residents and management to maintain a peaceful living environment</li></ul>
