What You'll Do At BCCS:<ul><li>
Supporting families to prevent the use and misuse of drugs and the development of substance use disorders</li><li>
Promoting child safety by identifying substance use and misuse in families and connecting families to interventions or treatment programs that will help the families identify, improve, and change unsafe behaviors so children can be safely cared for in the home.</li><li>
Reducing the risk of future harm to children by addressing the substance use needs in families.</li><li>
Transports clients to appointments in community and BCCS appointments.</li><li>
Provides liaison, support and educational services to DFS staff</li><li>
Participates in case presentation at weekly meetings</li><li>
Participates in client screening process prior to case management.</li><li>
Provides outreach activities to the community</li><li>
Administers various screening tools to potential clients and explains the nature of the program and the necessary rules.</li><li>
Assists clients in obtaining admission, entitlements, and related benefits.</li><li>
This is a primarily community-based position, so work will be performed outside of an office setting on a regular and consistent basis up to 100% of the time</li></ul>
Knowledge, Skills and Abilities:<ul><li>

Requirements:

 Qualifications for the position are:<ul><li>

What you will be doing? 
The Case Manager is responsible for providing confidential addiction guidance, education and support to individuals, families, and the community, as well as promoting healthy lifestyles and healthy choices. Under the supervision of the Director of Clinical Services, and in collaboration with a multi-disciplinary treatment team, the Case Manager ensures post treatment placement to the most appropriate level of care as recommended for each client. The goal of the case manager is to help clients re-enter society and re-engage in their regular lives as soon as possible. They may help clients accomplish this goal by coordinating care with external providers and community resources such as: Family Members, Primary Care Physicians, Psychiatrists, Intensive Outpatient, Partial Hospitalization, Legal Aids, Family Interventionists, University and High School Administrative Staff, and Employee Assistance Program Representatives. They may also help by assisting them with practical needs like finding housing, helping them find employment and arranging for continuing outpatient treatment after being discharged from the residence, if needed. 
 
What tasks are required?

<li>Job Type: Full Time; hours determined by Facility

<li>Developing the client discharge/transfer plans to ensure movement to the appropriate levels of care.
</li>
<li>Contacting referral sources, providing case consultation and coordination with referral sources (for example: business development partners, mental health treatment providers, criminal justice agencies, schools, employers, Child Protection Services)
</li>
<li>Coordinating ongoing care for client housing and therapeutic needs with the BriteLife Recovery team, substance abuse treatment providers, healthcare providers, community resources, and family members.

<li>Documenting all case management service notes, assessments, reassessments, referrals and follow-up in the client's clinical record, providing appropriate signatures and dating of such entries, including those made in electronic records.

<li>Positively promoting BriteLife Recovery to prospective clients, employees, and outside referral sources.

<li>Obtaining previous records that are relevant to the current treatment episode.
</li>
<li>Collaborating with the client in order to develop a written aftercare plan that is client-centered and recovery oriented, including upcoming appointments and contact information for recommended resources.

<li>Providing active case consultation with the multidisciplinary team.

<li>Attend individual supervision with supervisor to discuss growth and other client concerns or needs.

<li>Under direction of supervisor, establish appropriate therapeutic boundaries, utilization of identified clinical resources to review boundaries, limit setting for behavior, and develop therapeutic relations with clients.

What we need from you?

About the Role: The Case Manager provides comprehensive case management and support services to clients participating in substance abuse treatment programs. This position helps individuals access needed medical, psychological, social, educational, and vocational services to promote recovery and long-term stability. <div>Concern Housing is seeking a Case Manager to be responsible for providing case management services. The case manager is considered a resource in that they should be familiar with community resources and service to which they may help their clients gain access. The case manager is also an advocate for their clients.
Days/Hours: Monday - Friday; 4pm - 12am
Duties and Responsibilities:
<ul><li>Provide case management services (service planning, medication monitoring, assistance in forming linkages and/or referrals to clinical services, programs, vocational services and opportunities, community integration, entitlements etc.)</li>
<li>Provide individual counseling and support services; responsible for all documentation.</li>
<li>Housing services including assistance with maintaining their voucher, education assistance with the life skills needed to maintain an apartment and resolving maintenance issues.</li>
<li>Provide unit visits weekly and unit inspections monthly.</li>
<li>Participate in resolution of landlord and lease issues and coordination with public housing authority recertification.</li>
<li>Complete all necessary documentation to ensure compliance with funding requirements and agency quality assurance standards.</li>
<li>Attendance at weekly supervision and staff meetings.</li>

<li>Remain current with all Agency required compliance documents and trainings.</li></ul> 

Requirements: Bachelor&rsquo;s Degree in Psychology, Rehabilitation, Social Work or other related mental health discipline, plus 2 years experience working with persons with histories of substance abuse and/or mental illness. Previous work with Veterans preferred. The individual in this position must have good verbal and written communication skills as well as the ability to make sound judgments regarding client care.

Qualifications:</li>

Education:

Bachelor's degree - from an accredited institution with a major in chemical dependency, psychology, social work, counseling, specialty in the human services or another related field preferred.
