Job Summary
We are seeking a dedicated and compassionate Attendant/Home Health Provider to deliver high-quality in-home care to our clients. The ideal candidate will possess a strong background in patient care, with experience supporting individuals with diverse needs, including those with developmental disabilities, dementia, or requiring hospice care. This role offers an opportunity to make a meaningful difference in the lives of our clients by providing attentive assistance with daily activities and health monitoring.

Responsibilities

  • Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support.
  • Monitor and observe patients’ health status, including vital signs and overall well-being.
  • Support medication administration and ensure proper documentation in electronic health record (EHR) systems while maintaining HIPAA compliance.
  • Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques.
  • Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively.
  • Assist with meal preparation, feeding, light cleaning, laundry, and maintaining a clean living environment.
  • Document patient progress accurately in medical records and communicate effectively with healthcare teams.
  • Follow all safety protocols during patient care activities and participate in ongoing training such as CPR certification.

Skills

  • Strong patient observation skills to detect changes in health status promptly.
  • Experience working in assisted living facilities, nursing homes, or home health environments.
  • Ability to assist with complex tasks such as catheter care, hospice support, and dementia-specific needs.
  • Competence in vital signs measurement and medication administration procedures.
  • Excellent communication skills to interact empathetically with clients and their families.
  • Prior long-term care experience working with individuals with disabilities or developmental challenges is preferred.
