Crossroads Campus was founded in 2010 and is an IRS-designated 501(c)(3) serving Nashville, Tennessee. Our mission is to provide opportunities for healing and developing resilience to young people facing barriers and abandoned pets in order to transform human and animal lives.

We provide trauma-informed supportive housing and workforce development opportunities for young adults, helping them build independence across key life domains including employment, education, housing, and personal wellbeing.

POSITION SUMMARY

Reporting to the Program Director, the Senior Case Manager is a leadership role responsible for supervising the case management team while maintaining a small caseload of 6–8 young adults. This position ensures the consistent delivery of high-quality, trauma-informed case management services and the timely, accurate completion of all program documentation.

The Senior Case Manager serves as a coach and resource for Case Managers, ensuring alignment with program standards, organizational goals, and best practices.

PRIMARY DUTIES AND RESPONSIBILITIES

Leadership & Supervision

  • Supervise, coach, and support Case Managers in delivering effective, trauma-informed services
  • Provide regular weekly one-on-one supervision, feedback, and performance support to ensure quality and consistency
  • Monitor staff caseloads, workflow, and service delivery
  • Support onboarding and training of new Case Managers
  • Promote a culture of accountability, collaboration, and continuous improvement

Program Oversight & Quality Assurance

  • Ensure all case management documentation is completed accurately and on time, including:
  • Daily Living Assessments (DLA-20)
  • Individual Progress Plans (IPP)
  • Case notes and service tracking
  • Conduct regular audits of case files and documentation systems
  • Ensure compliance with internal standards and funder requirements
  • Support data collection, reporting, and program performance tracking
  • Collaborate with leadership to meet program goals and outcomes

Direct Case Management (Limited Caseload: 6–8 Young Adults)

  • Build trusting relationships with assigned young adults
  • Assess strengths, needs, and goals across key life domains
  • Develop and regularly update Individual Progress Plans (IPP)
  • Connect participants to community resources and services, including:
  • Healthcare and mental health services
  • Employment and education supports
  • Housing stability resources
  • Financial literacy and life skills
  • Support young adults in navigating systems and building independence
  • Provide crisis support and coordination as needed (non-clinical)

Program Collaboration

  • Work closely with program staff to ensure coordinated, holistic service delivery
  • Maintain and utilize an up-to-date network of community resources
  • Support group programming, workshops, and life-skills training as needed
  • Participate in team meetings and cross-functional collaboration

REQUIRED QUALIFICATIONS

  • Bachelor’s degree in Social Work, Psychology, or related field required; Master’s preferred
  • 3–5+ years of experience in case management or young adult services
  • Prior experience supervising or leading staff strongly preferred
  • Strong knowledge of trauma-informed care and young adult development
  • Experience coordinating community resources and navigating complex systems
  • Valid driver’s license and ability to transport clients as needed
  • Exceptional skills in engaging and assessing young adults and coordinating
  • Ability to lift, push, pull at least 15 lbs.

COMPENSATION

Salary range is $60,000-$65,000 depending on experience.
