Full time
Purpose: We are seeking a compassionate and skilled Senior Nurse Navigator to lead patient care initiatives, ensuring seamless healthcare experiences for our patients. This role involves comprehensive patient assessment and coordination of care across multidisciplinary teams, particularly for patients participating in bundled payment programs.
Responsibilities:- Plan and deliver professional nursing services in alignment with established policies and procedures.
- Collaborate with the care team to assess and manage patients throughout their healthcare journey, ensuring smooth transitions from inpatient to post-acute care.
- Facilitate interdisciplinary care coordination rounds to gather clinical information and update discharge plans.
- Maintain accurate documentation of patient assessments, care plans, and transitions of care in appropriate systems.
- Establish partnerships with community providers and post-acute facilities to enhance patient outcomes and satisfaction.
- Provide mentorship and guidance to junior nursing staff, promoting professional development and shared governance.
- Maintain compliance with safety protocols and effectively utilize personal protective equipment (PPE).
- Adapt to shifting priorities and manage multiple tasks in a fast-paced environment.
Minimum Qualifications:- Graduation from an accredited school of nursing.
- Valid RN licensure in the applicable state(s) of practice.
- Valid driver’s license where required by assignment.
- Strong verbal and written communication skills, along with advanced analytical and organizational abilities.
Preferred Qualifications:- Bachelor of Science in Nursing (BSN) degree.
- Specialty certification relevant to clinical nursing practice area.
- 3-5 years of case management or clinical experience in a hospital or community-based setting.
Pay Range: $31.50-$45.70
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
