SENIOR SERVICES CASE MANAGER

Overview

Provides direct services to support seniors within the Taos Pueblo Community in navigating the various support systems available. Assesses client needs, assists with gaining access to necessary resources, and promotes communication between the elder, their family and other caregivers to promote effective care and social support for the client. Works closely with various agencies, healthcare providers, and other programs to coordinate services for seniors. Assists seniors with obtaining and completing appropriate forms.

ESSENTIAL FUNCTIONS:

  • Accepts referrals from the Taos Pueblo Public Health programs, Indian Health Services, Taos Department of Public Safety and the community at-large.
  • Contacts eligible clients to extend support services and available resources; assists clients in enrolling for services in local, state and federal benefit programs that will support and contribute to their respective health and well-being.
  • Builds rapport with the elder and their family, friends, and other caregivers in order to build effective support networks; promotes open lines of communication between clients, service providers and, where appropriate, clients' family members.
  • Arranges home visits and provides feedback to family members and other caregivers to ensure elder receives effective care; educates family members on how to deal with challenging behaviors from patients with dementia or other conditions affecting an individual's mental state;
  • Assesses client's social needs and assists elders with developing long-term care plans to address needs; conducts assessments of eider's abilities to make competent decisions regarding care and assists them with navigating the legal process if they are unable to make decisions for themselves.
  • Coordinates services with physicians, therapists, nurses and other healthcare professionals; provides case management services to elders who are discharged from hospitals or nursing facilities and assists elders; may occasionally transport clients to medical appointments, as needed.
  • Assists elders in managing their finances and applying for government benefits, such as Social Security Disability Insurance (SSDI), Medicare, Medicaid, and Veteran's programs.
  • Documents and obtains information pertinent relating to client's progress in cases where care is received outside the client's home; collects and maintains statistics to demonstrate program compliance and compiles monthly and quarterly narrative and statistical reports, as needed.
  • Maintains a comprehensive listing of all elders residing in the immediate Taos Pueblo Community in case of emergent issues requiring response, such as loss of power, blizzards, fires, etc.; develops and maintains a comprehensive resource list for distribution to elders of community resources available to them.
  • Advocates for elder inclusivity and access throughout the community; assists with the development and delivery of effective programming at the Senior Center; participates in Division-led health and community wellness activities and may coordinate community educational sessions for Seniors.
  • Performs other duties as assigned.

  • Ability to assist seniors with accurate completion and submission of paperwork for eligible benefits.
  • Ability to conduct effective needs assessments to identify resources and services that will aid the client's health and well-being.
  • Ability to analyze and solve problems.
  • Ability to maintain confidentiality.
  • Ability to use a personal computer and to navigate job-specific databases and software.
  • Ability to handle multiple tasks and meet deadlines.
  • Ability to compile statistical and narrative reports.
  • Ability to work effectively with other agencies and organizations.

WORK ENVIRONMENT:

Work is regularly performed in the field and/or an office setting with a low noise level. Evening and/or weekend work may be required.

MINIMUM QUALIFICATIONS:

High School Diploma or GED and four years of experience in senior services, community services or other related field of work. Must be able to pass drug test and background check, with NO prior convictions of any felonies.

Pay: $22.44 - $29.41 per hour
