Skilled Home Health LVN Field Nurse

Mission Home Health provides a wide array of healthcare services to patients in the San Antonio, Texas Metropolitan Area. Our focus is to provide patients and their families the opportunity to receive needed medical care while remaining in the comfort and security of their homes. Our services include nursing, PT, OT and Speech Therapy services as well as Social Work and Home Health Aides.

Role Description

This is a full-time hybrid role as a Licensed Vocational Nurse (LVN) with Mission Home Health in the San Antonio, Texas Metropolitan Area. The LVN will provide quality, comprehensive nursing care to assigned patients in a home setting. While most of the work is done at the patient's home, some remote work is acceptable.

Qualifications

  • Valid Texas LVN License or the ability to obtain LVN license in the state of Texas
  • Minimum of one year of LVN Nursing experience and preferably with previous home health experience
  • Excellent verbal and written communication skills
  • Able to work independently while maintaining a professional presence with patients and their families
  • Ability to take direction and be a team player
  • Able to lift a minimum of 50 pounds
  • Valid Texas driver's license, reliable transportation and proof of current auto liability insurance
  • Successful completion of a competency evaluation program or examination which meets state and federal guidelines for home health aides

Responsibilities

  • Provide direct patient care in home health settings, including catheterization, tube feeding, and IV infusion as needed.
  • Conduct thorough patient assessments and maintain accurate electronic health records (EHR).
  • Ensure compliance with HIPAA regulations while delivering compassionate care.
  • Collaborate with healthcare teams to develop and implement individualized care plans.
  • Communicate effectively with patients and families to educate them about health management and treatment options.
  • Drive to patient locations, ensuring timely and reliable service.

Pay: $33.00 - $35.00 per hour
