SKILLED NURSING - RN TRANSITION NURSE

Skilled Nursing - RN Transition Nurse - (Mon-Fri, 10:00 AM to 6:30 PM) - RN is responsible for the oversight of and participation in the admission/discharge process, including the screening of potential admissions for appropriate diagnosis, ability to participate in the rehabilitation program, and for successful outcomes following therapy. The RN will work closely with our physician groups and specialists, and the interdisciplinary team involved in the care of the residents. This RN will collaborate with home care and resident support services prior to discharge. The RN may be required to assume charge nurse duties as needed.

Requirements - Minimum 3 years experience as a professional nurse is required. Current and valid licensure by the Commonwealth of Pennsylvania as a Registered Nurse, as well as current CPR certification, are required, and the ability to communicate with multiple disciplines in a demanding environment.

Rate~ $37.22-$44.67 an hour.

Hours~ 10:00am-6:30pm, M-F
