SNFist Nurse Practitioner (NP) or Physician Assistant (PA)

About the Role

We are seeking an experienced and motivated SNFist Nurse Practitioner or Physician Assistant to provide on-site medical rounding services at two Skilled Nursing Facilities in Palm Springs, CA.

This role focuses on post-acute and long-term care management, care coordination, reducing hospital readmissions, and ensuring high-quality clinical outcomes for medically complex patients.

You will work collaboratively with facility staff, case managers, specialists, and families to deliver comprehensive, patient-centered care.

Responsibilities

• Perform routine and acute patient rounds at 2 SNF locations
• Conduct admissions, readmissions, follow-ups, and discharge visits
• Manage chronic and acute medical conditions
• Order and interpret labs, imaging, and diagnostic testing
• Complete timely and compliant documentation in EMR
• Collaborate with interdisciplinary teams (DON, social work, therapy, case management)
• Participate in care plan meetings as needed
• Ensure compliance with CMS and regulatory standards
• Focus on reducing avoidable hospital readmissions

Qualifications

• Active California NP or PA license (required)
• Board Certification (FNP, AGNP, or PA-C)
• DEA License (required)
• SNF / Post-Acute / Geriatric experience preferred
• Strong clinical assessment and decision-making skills
• Comfortable managing medically complex patients
• Ability to work independently

Schedule Options

• 2-3 days per week (Flexible scheduling)
• Typical rounding hours: approx.
• No weekends (optional depending on coverage model)
• Minimal call, if applicable)
