Responsibilities

Provides continuous supervision and case management services to assigned consumers on caseload
Assists consumers with their activities of daily living skills, budgeting, and linkage to applicable benefits
Provides consumers with medication education and observes their self-administration of medication
Must adhere to the agency’s driving policy of transporting consumers in a safe and responsible manner
Provides input to treatment and service plans as well as their implementation
Must be able to document all relevant information to the consumers' electronic chart records

Qualifications

BSW in social work, BA/BS in psychology, criminal justice, sociology, or a related field
BA in an unrelated field with two years of previous experience
AA in a related field with two years of previous experience
Previous experience desired
Valid Driver’s License

Monday through Friday, 12:30am - 8:30am
