Social Services Case Manager

DSSI is hiring Case Managers to work with a workforce services company in the Escondido area. In this position you will play an integral part of helping the surrounding community access resources to help eliminate barriers. The team is committed to providing their very best and have been crucial in the success of assisting families reach self sufficiency.

Details:

• Workdays: Monday-Friday
• Work Hours: 8am-5pm
• Pay Rate: $25/HR

Duties:

• Meets with participants to identify barriers and challenges to employment/self-sufficiency and develop strategies to manage these challenges through motivational interviewing techniques.
• Maintains complete, accurate and timely case notes and related records in manual and electronic systems.
• Track and reports out on participant progress and participation in assigned employment services activities
• Determine eligibility and/or issue supportive services such as transportation, child care and ancillaries to support participants in employment service activities
• Outreach to re-engage participants who fail to show for appointments and assigned activities
• Create, update and ensure completion of plans according to contract deadlines.
• Assess and refer participants to appropriate departments and/or agencies for job readiness, training, support services, specialized services, and/or behavioral health services
• Attends scheduled supervision meetings to discuss participants’ progress.
• Utilize county and company computer data processing systems to efficiently track and record participant activity
• Complete all necessary case management documentation and forms; maintain records and files; and schedules appointments
• Other duties as assigned
• Bilingual/Spanish a plus
