• Social Service Designee Pay Rate: $38-$43 per hour, paid weekly
• 24/7 on call representative from Interim Healthcare

Position Summary

The Social Services Designee is responsible for supporting the psychosocial well-being of residents in a skilled nursing or long-term care facility. This role ensures residents’ social, emotional, and discharge planning needs are met in accordance with federal regulations (F-Tags) and facility policies.

Key Responsibilities

• Conduct psychosocial assessments upon admission and quarterly/annual reviews
• Develop and update care plans addressing residents’ emotional, social, and discharge needs
• Participate in interdisciplinary team (IDT) meetings and contribute to care planning
• Coordinate and facilitate discharge planning, including referrals and community resources
• Assist residents and families with adjustment to placement, crisis intervention, and grievance resolution
• Maintain compliance with CMS regulations (including F-Tag 679, 680, 684) and state guidelines
• Complete required documentation in EMR/MDS support as applicable
• Advocate for resident rights and ensure dignity, privacy, and quality of life
• Support behavioral and mental health needs in collaboration with nursing and providers
• Assist with advance directives, guardianship, and care conferences

Qualifications (North Dakota Typical Standards)

• Bachelor’s degree in Social Work, Human Services, Psychology, or related field preferred
• May serve in this role without a social work license if qualified by experience (per facility policy and CMS guidelines)
• Previous experience in long-term care, case management, or social services preferred
• Knowledge of MDS, care planning, and discharge coordination strongly preferred
• Strong communication, documentation, and organizational skills

Licensure (if applicable)

• LBSW/LMSW preferred but not always required for Social Services Designee roles in North Dakota LTC settings
