Pay: $70,000 – $80,000 annually, negotiable based on experience

Required Qualifications – Must Meet All

  • Bachelor’s degree in Social Work or Human Services
  • Minimum of 1 year of social services experience
  • Knowledge of discharge planning, care coordination, and psychosocial assessments
  • Understanding of resident rights and advocacy in a healthcare setting
  • Strong communication, documentation, and organizational skills
  • Ability to work in a fast paced skilled nursing or healthcare environment

We are seeking a dedicated Social Services Director who brings both heart and professionalism to the role and understands the importance of advocacy, discharge planning, emotional support, and interdisciplinary coordination within a Skilled Nursing Facility setting.

Victoria Post Acute Care is a 120 bed skilled nursing facility located in El Cajon, California, committed to delivering exceptional care through compassion, teamwork, and clinical excellence. Our team proudly serves residents across long term care, short term rehabilitation, and memory care, with a strong focus on dignity, quality of life, and individualized support.

We are currently seeking an experienced Social Services Director with skilled nursing or healthcare experience who is highly organized, emotionally intelligent, and passionate about helping residents and families navigate the care journey with confidence and support.

Position Summary

As the Social Services Director, you will play a vital leadership role in enhancing the psychosocial well being of our residents while serving as a trusted resource for families, staff, and the interdisciplinary team. You will oversee social services functions within the facility, support resident advocacy, assist with care transitions and discharge planning, and help ensure each resident receives thoughtful, person centered care.

This role is ideal for someone who thrives in a fast paced healthcare environment, leads with compassion, communicates with professionalism, and understands how to balance compliance, empathy, and resident centered service at a high level.

Key Responsibilities

Oversee and coordinate the facility’s social services program to support the emotional, social, and psychosocial needs of residents
Conduct resident assessments and participate in care planning to ensure individualized support and services
Assist residents and families with adjustment to the facility, transition planning, discharge needs, and community resource coordination
Serve as a liaison between residents, families, staff, and outside agencies to promote continuity of care and positive outcomes
Support resident rights, advocacy, and grievance processes with professionalism and compassion
Collaborate closely with nursing, therapy, admissions, case management, and administrative teams
Maintain accurate and timely documentation in accordance with facility, state, and federal requirements
Help support a positive resident experience by addressing concerns, strengthening communication, and fostering trust with families
Ensure compliance with all regulatory standards related to social services in a skilled nursing environment

Qualifications

Bachelor’s degree in Social Work or Human Services required
Minimum of 1 year of experience in social services required
Skilled nursing facility or healthcare experience highly preferred
Knowledge of discharge planning, care coordination, resident rights, and psychosocial assessments
Strong communication, documentation, and organizational skills
Ability to lead with empathy while maintaining professionalism and regulatory awareness
Compassionate, dependable, and team oriented with a genuine commitment to resident care
