Social Worker Responsibilities:

• Assess, develop, and implement individualized care plans for residents, taking into account their physical, emotional, and social needs

• Facilitate communication between residents, families, and healthcare team members to ensure that resident needs are met

• Provide emotional support and counseling to residents and their families, addressing issues related to anxiety, depression, grief, and loss

• Conduct social assessments and make recommendations for community resources and services to support residents' needs

• Collaborate with healthcare team members to develop and implement care plans that address social and emotional needs

• Maintain accurate records and documentation of social worker activities and resident progress

Social Worker Requirements:

• Bachelor's degree in Social Work, Psychology, or related field (or equivalent experience)

• Prior experience as a Social Worker (or equivalent experience)

• Prior experience in a skilled nursing facility

• Strong understanding of social worker principles and practices

• Excellent communication, assessment, and intervention skills

• Ability to work effectively in a fast-paced environment and prioritize multiple tasks

• Compassionate and empathetic approach to working with residents and families
