Division: Within the Social Services Department at ELMHS, Located in Jackson, LA

The Mission of ELMHS is to provide mental health evaluation, treatment, habilitation, and consultation for adults. Programs include inpatient interventions, jail-based evaluations, supervised group home and residential programs, forensic evaluation, care, custody, and aftercare; intermediate and long-term psychiatric services are coordinated with the total system of care in Louisiana

ELMHS is seeking a passionate Social Worker to deliver client-centered services, including assessment, case management, crisis intervention, and community resource coordination. The ideal candidate will have experience working with diverse populations facing mental health challenges, trauma, or crises. This position will function as the department head for the ELMHS Social Services department.

Minimum Qualifications

Possession of a current Louisiana license to practice as a Licensed Master's Social Worker (LMSW) or a Licensed Clinical Social Worker (LCSW) plus:

A master's degree in social work followed by five years of social services experience.

Job Duties and Other Information

Job Duties:

In this role, you will:

  • Assign appropriate staff to work assignments, monitor for accurate completion of all documentation and assigned tasks within stated deadlines, and approve requested leave within ELMHS and LDH guidelines.
  • Monitor and direct subordinate schedules to ensure adequate coverage is available within the Social Service Department locus of control, as well as cover subordinates’ unit duties, tasks, or caseload, when necessary.
  • Review and write contracts with outside service providers such as Deaf Focus, professional schools, to include but not limited to, LSU, SUNO, SU, and SLU Schools of Social Work.
  • Provide Board Approved Clinical Supervision, BACS, for those Social Workers seeking the LCSW credential, if credentialed.
  • Collect/integrate social assessment information from patient, family, etc. into a complete social assessment within agency delineated timelines, provide for ongoing monitoring of patients’ commitment status, and develop treatment plans with the patient and treatment team and provide individual, group, or family clinical interventions
  • Proactively refer and locate in cooperation with the Regional providers and the SMO resources and services used for follow-up for those persons ready for discharge to the community.
  • Assist with efforts in coordinating and locating suitable Permanent Supportive Housing, placements in nursing, group, or licensed boarding homes for those patients unable to return home or require placement, and ensure the department follows governing body and SMO mandates for discharge process and referrals.
  • Complete all LDH/OBH/Continuity of Care (COC) aftercare referral documentation for the chart and agencies’ needs and document all aftercare appointments and other referrals on discharge forms, progress notes, and COC/OBHIIS database as needed.
  • Through the Department peer audit monitoring performance improvement process, ensure the development of the Master Treatment Plans uses the SMART method, and the chart documentation uses the BIRP methodologies.
  • Monitor the timeliness and content of clinical processes to include psychosocial assessments, treatment planning, clinical interventions, and chart documentation, which are conducted as stipulated in the guidelines found in the ELMHS Social Services Departmental meetings on a monthly basis, with a follow-up plan for improvement based on the data gathered.
  • Maintain the Social Services Department folder resource list and the OBHIS provider database and troubleshoot with OBH as needed.

Work Schedule: Monday - Friday

Compensation: This position has an SER of $38.14 an hour or $3051.20 bi-weekly. This position is eligible for Longevity Pay starting at $2.00 Per Hour Physically Worked
