The Clinical Social Worker Case Manager II provides psychosocial resources and support to individuals with acute, chronic or terminal illnesses. This role helps manage and plan the patient's treatment plan, providing assistance through knowledge, expertise and advice. This role connects the patient with resources that help achieve a targetable goal.
Essential Job Function
- Provides case management to patients to meet current and future psychosocial and medical needs.
- Completes psychosocial assessments and analysis for patients to help the patient better understand what treatments would fit their means.
- Comforts patients, providing emotional support to the patient to make the patient feel at ease, enabling them to more easily move through the healthcare system, providing support at all stages.
- Provides case management services to individuals living with HIV in their community.
- Engages Cone partners to create a system/plan for success.
- Serves as a resource to local and state partners.
- Serves patients who are living with HIV to give them the highest possibility chance of positive health outcomes while also understanding what treatment is realistic for them.
- Utilizes various counseling and therapy techniques to help place the patient at ease with regards to clinical work and the patients health condition, being caring yet honest.
- Performs other duties as assigned.
Education
- Required: Masters of Social Work from an accredited institution
Experience
- Required: Minimum of two (2) years of recent experience in a health-related environment
Licensure/Certification/Listing
- Required: Valid Drivers License (if driving a Cone Health vehicle, must have 5 years of driving experience and Motor Vehicle Record (MVR) must be approved by Risk Management)Licensed Clinical Social WorkerBLS certification
