Job Overview

We are seeking a compassionate and dedicated Social Worker to join our healthcare and community services team. The ideal candidate will provide comprehensive support to individuals and families, addressing a wide range of mental health, behavioral, and social needs. This role involves working with diverse populations, including children, adults, and seniors, in various settings such as hospitals, community centers, and clients’ homes. The Social Worker will play a vital role in facilitating recovery, improving quality of life, and promoting overall well-being through evidence-based interventions and compassionate care.

Duties

  • Conduct thorough patient assessments to identify social, emotional, and behavioral needs using clinical counseling techniques.
  • Develop individualized care plans incorporating behavioral therapy, cognitive behavioral therapy, and motivational interviewing strategies.
  • Provide crisis intervention and crisis management to stabilize individuals experiencing mental health emergencies or trauma.
  • Offer child & family counseling, including working with children with autism, developmental disabilities, or PTSD.
  • Deliver specialized care such as Alzheimer’s care and addiction counseling tailored to each client’s circumstances.
  • Coordinate services within home & community care settings to ensure clients receive appropriate support and resources.
  • Maintain accurate medical documentation, records, and case notes in compliance with HIPAA regulations using EMR systems.
  • Collaborate with healthcare providers, case managers, and community organizations to facilitate comprehensive patient care.
  • Supervise or mentor junior staff or interns as part of leadership responsibilities when applicable.
  • Conduct diagnostic evaluations and participate in group therapy sessions for diverse client populations.
  • Engage in data collection for program evaluation and quality improvement initiatives.

Qualifications

  • Bachelor’s or Master’s degree in Social Work (BSW or MSW) from an accredited institution.
  • Valid licensure or certification as required by state regulations for clinical social work practice.
  • Extensive experience in behavioral health, mental health counseling, or related fields such as hospital experience or home & community care.
  • Proficiency in medical terminology, medical records management, and medical documentation standards.
  • Strong skills in crisis intervention, crisis management, diagnostic evaluation, and case management.
  • Knowledge of applied behavior analysis (ABA), behavioral therapy techniques, and working with individuals with autism or developmental disabilities.
  • Experience with EMR systems and data collection tools for clinical documentation and patient tracking.
  • Excellent communication skills for individual/group counseling sessions and interdisciplinary collaboration.
  • Ability to handle sensitive situations with professionalism while maintaining confidentiality per HIPAA guidelines.
  • Leadership qualities with supervising experience preferred for senior roles or team management responsibilities.
