Social Worker (LMSW), $60,000.00 + Benefits

Caring Angels Home Care is looking for a full-time compassionate Social Worker (LMSW) for our Rehabilitation Center in Egg Harbor New Jersey. As Social Worker (LMSW), you will play an integral role in the daily functions of our Center.

LMSW Responsibilities:

• Guide, asses and communicate psychosocial status to the RN Case Manager and interdisciplinary group.

• Connect and evaluate medical needs, home situations, financial resources, and community support.

• Implement personalized social work plans and collaborate with your Bereavement Coordinator.

• Shape care plans, attend interdisciplinary meetings, and address psychosocial stresses.

• Provide empathetic counseling to patients and families.

Qualifications:

• Required: Master of Social Work from an accredited school of social work.

• Required: Licensed as a Social Worker from the State Board of Social Work

Experience:

• Preferred: Minimum one (1) year experience as a social worker in a rehabilitation setting.

• Required: Minimum one (1) year experience as a social worker in a healthcare setting; long term care or medical surgical/acute care setting preferred.
