Job Summary:
We are seeking a compassionate and dedicated Social Worker with a Master of Social Work (MSW) degree to join our team. The Social Worker will be responsible for assessing residents' needs, developing and implementing treatment plans, and providing support and resources to help clients improve their overall well-being.

Pay Range: $72,800 - $95,000 per year based on experience and qualifications

Qualifications:

• Master of Social Work (MSW) degree from an accredited institution
• At least 2 years of experience in healthcare, LTC/SNF, geriatrics, behavioral health, or case management
• Strong knowledge of social work theories, interventions, and techniques
• Demonstrated ability to manage complex psychosocial cases, including dementia-related needs and family dynamics.
• Working knowledge of community resources and benefits navigation.
• Excellent communication and interpersonal skills
• Ability to work effectively with individuals from diverse backgrounds

Responsibilities:

• Conduct assessments to determine residents' needs and develop individualized treatment plans
• Provide counseling and support services to residents' facing various challenges
• Connect residents with appropriate resources, such as mental health services, housing assistance, and employment programs when discharging
• Collaborate with other healthcare professionals to ensure comprehensive care for residents
• Maintain accurate and confidential resident records in compliance with regulations
• Participate in care conferences and team meetings to review resident progress and adjust treatment plans as needed
• Stay informed about community resources and programs to better assist clients in accessing the support they need
