PACE Central Michigan is opening a new facility in Houghton Lake—and we’re seeking an MSW who is ready to help build something meaningful in a rural community.
This opportunity is ideal for an MSW seeking more than a traditional MSW role—someone who is eager to help build a new PACE Center from the ground up and play an active role in shaping care delivery at our Houghton Lake location.
Position Summary
Responsible for the direct social work case management services to the participants of the PACE Central Michigan program. Works under the direct and indirect supervision of the Director of Care Coordination or designee, but is independent in the application of advanced social work knowledge and skills. Provides such social services support as participant screening, case management, counseling, and referral.
What You’ll Do
- Participates in the Interdisciplinary Team's initial assessments, care planning, and periodic re-assessments (minimally every 6 months) of the participant's case. Assessments are to be completed prior to the scheduled team meeting. Communicates participant changes to team members.
- Maintains current written case management records, including periodic reassessments of program participants.
- Coordinates 24-hour care delivery. Provides individual and family counseling, develops and leads group counseling and activities.
- Refers participants and families to appropriate community agencies or facilities, acts as liaison with such organizations, and as advocate for participants.
- Encourages effective relationships among staff geared to team building and maintenance of a cohesive team.
- Consults with and advises staff members as to the relationship of social, emotional, and cultural factors to health and medical care, and as to the availability of social services in the community.
- Participates in Program and Policy development of Social Work component.
- Facilitates enrollment and disenrollment from PACE Central Michigan.
- Facilitates Medicaid application and redetermination process with Michigan Department of Human Services.
- Documents participant changes appropriately in the medical record.
- Demonstrates active participation in QI processes.
- Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning.
What We’re Looking For
- Master’s Degree from an accredited School of Social Work.
- Minimum of 2 years’ experience in a health-related area.
- Minimum of 1 year of experience working with the frail elderly population.
- LLMSW (with ability to obtain full licensure prior to limited license expiration) or LMSW, valid MI Drivers License, and proof of vehicle registration and insurance.
- CPR certification or willingness to obtain upon hire.
- Individual therapy skills for geriatric participants, analytical thinking and problem-solving abilities.
- Meet a standardized set of competencies for the specific position description established by PACE Central Michigan and approved by CMS before working independently.
