- Admission, discharge, department transfer and patient referrals
- Assess the total needs of patients and prepare plan of nursing care.
- Carry out efficient nursing care, administration of drugs and treatment and monitoring for side effects and reactions.
- Responsible patient’s diet.
- Assist Doctor’s in examination of patients and treatment process.
- Collecting, labelling and dispatch of specimens.. Co-ordinate and help with clinical investigations/diagnostics test & procedures
- Demonstrate and carry out preoperative and post-operative care of patients.
- Care of patient’s personal effects in accordance with hospital rules.
- Follow prescribed rules in case of accident or death of a patient.
- Give health education to patients and their attendants.
- Intimation to nursing supervisors of any emergency or unusual occurrence in the ward, Incident reporting.
- Responsible to maintain all medo -legal policy and protocol
- Prepare dressing, drug, crash trolleys
- Support Junior nursing staff in nursing care and documentation part
- To ensure Mentor / mentor body system is following while allocating patient by the team leader .
- Ensure proper ISBAR record / Endorsement tool kit is followed while each handover by the allocated staff.
- Make sure the inter departmental transfer of patient is happening in a safe manner.
- Make sure all vulnerable patients are identified and the proper assessment / reassessment is done at periodic intervals.
- Patient required indents / refunds both drugs / medical consumables are to be done in a safe and secure manner.
- Storage of medications to be cross checked for manufactures' recommendations including fridge medications
- Ensure the Crash cart is maintained with a uniform pattern of check list in all areas and the used items are replenished in a timely manner.
- Ensure all incident reports of near misses, medication errors & adverse events are routed to Quality department through proper channel for analysing RCA & CAPA.
- Make sure the correct protocol as defined is following by the staff while on verbal order.
- Make sure correct medication rights are following by the staff during medicine administration.
- Identify malfunctioning equipment or devices and inform charge nurse
- Ensure all equipment’s or devices are properly stored after use.
- Maintain quality assurance of nursing documentation and Patient records including nurses' note, all assessments & reassessments.
- Pain management and assessment must screened at the time of admission and reassessment is done in timely manner.
- To be familiar with LOOK ALIKE and SOUND A LIKE medicines , narcotic Policy & storage of medications including fridge medications .
- Adhere to standard precautions and hand hygiene guidelines while handling indicated patients.
- Adhere to safe linen management practices , cleaning, disinfection and sterilization practices, handling, storage and disposal of bio medical waste , Isolation (barrier and reverse barrier nursing) practices while on nursing care , processes for sorting, labelling, handling, storage, transporting and disposal of hazardous materials
- Co-ordinate and facilitate work of other staff, e.g. therapists, dietitian, etc.
- Maintaining good inter personal relationship among all categories of staff and with patients and their relatives.
- Maintain cleanliness of ward. Proper upkeep and repairs of linen and ward equipment
- Maintain confidentiality of patients and ensure taking informed consents
- Plan and prioritize the nursing care plan according to patient condition.
- Daily check of emergency drugs, department supplies, stock registers, inventories.
- Maintains professional and technical knowledge by attending educational workshops participating CNEs and in – service education programs.
