- Conduct initial and ongoing patient assessments, including vital signs, medical history, and overall patient condition.
- Administer medications and treatments as prescribed by physicians or authorized medical professionals.
- Implementation of Care Plans
- Collaborate with the healthcare team to develop, implement, and evaluate individualized patient care plans.
- Monitor and document patient responses to treatments and interventions, adjusting care plans as necessary in collaboration with the supervising physician.
- Clinical Procedures & Documentation
- Perform clinical procedures such as wound care, intravenous line placements, catheterization, and specimen collection accurately and safely.
- Maintain thorough, accurate, and up-to-date patient records, ensuring confidentiality and compliance with hospital policies and regulatory standards.
- Patient & Family Education
- Educate patients and their families about specific conditions, post-procedure care, and medication administration.
- Provide guidance on health promotion, disease prevention, and lifestyle modifications when relevant.
- Team Collaboration
- Work collaboratively with doctors, senior nurses, and allied health professionals to ensure seamless, multidisciplinary care.
- Participate actively in handover meetings and team discussions to provide comprehensive patient information.
