Substance Use Disorder Case Manager

In this vital role, you will coordinate comprehensive case management plans, connect clients to treatment and supportive services, and provide ongoing support throughout their recovery journey. Duties

Conduct comprehensive client assessments to evaluate substance use history, behavioral health needs, and social determinants of health using trauma informed and client centered approaches.

Develop individualized case management plans that incorporate recovery goals, behavioral health strategies, and service coordination in alignment with program standards and confidentiality requirements.

Coordinate community based services including outpatient treatment, housing support, reentry services, medical and behavioral health providers, and other social service systems to ensure continuity of care.

Maintain accurate and timely documentation of intakes, referrals, outreach efforts, and client progress notes in compliance with HIPAA and program reporting requirements.

Provide client centered support by guiding individuals through available services, explaining care options in accessible language, and supporting engagement in recovery and stabilization goals.

Conduct regular client check ins and outreach including follow up on referrals and re engagement efforts for clients who are disengaged from services.

Collaborate with community partners including social service agencies, probation departments, treatment providers, and peer support networks to strengthen care coordination.

Support transition planning by connecting clients to long term resources that promote stability including housing, employment, education, and ongoing treatment services.

Experience

Prior experience in community based case management, behavioral health, substance use treatment, or reentry services is highly preferred.

Demonstrated knowledge of addiction counseling, harm reduction, and mental health support practices is essential.

Experience working with vulnerable populations including individuals impacted by incarceration, homelessness, or substance use is strongly valued.

Proven ability to manage caseloads, complete timely documentation, and track client progress effectively.

Familiarity with referral systems, community resources, and service coordination across multiple systems of care.

Strong understanding of HIPAA compliance, documentation standards, and case management best practices.

Background in social work, counseling, or related human services fields is advantageous.

Ability to build rapport, maintain professional boundaries, and engage clients with empathy and accountability.

Job Type: Full-time

Pay: $29.80 - $33.48 per hour
