SUD Case Manager

Position Summary

We are seeking a dedicated Substance Use Disorder (SUD) Case Manager to join our team. This role provides direct case management services to individuals with substance use disorders, including assessment, service planning, care coordination, and advocacy. The ideal candidate is empathetic, organized, and committed to supporting clients in achieving their recovery goals.

Key Responsibilities

  • Conduct comprehensive assessments to determine client needs and appropriate services
  • Develop individualized service and recovery plans in collaboration with clients
  • Coordinate care across a range of services including medical, mental health, housing, legal, and employment
  • Provide education, support, and advocacy to clients and their families
  • Monitor client progress, update service plans, and maintain detailed documentation
  • Collaborate with treatment providers, social workers, peer support specialists, and community partners
  • Participate in team meetings, supervision, and ongoing training
  • Ensure compliance with all relevant policies, procedures, and confidentiality standards

Qualifications

  • 1–2 years of experience in case management or working with individuals with SUD
  • Knowledge of harm reduction, trauma-informed care, and recovery-oriented systems
  • Strong communication, problem-solving, and organizational skills
  • Ability to work independently and as part of a multidisciplinary team
  • Valid driver’s license and reliable transportation (if travel is required)

Job Type: Full-time

Pay: $17.00 - $19.00 per hour
