Job Summary: The Supervisor, Clinical Hospitalist Care Facilitators guides the Hospitalist Care Facilitators (HCFS) to assess, plans, implement, and coordinate the discharge plan as directed by Hospitalist Program to maximize the Hospitalist's efficiency. This role maximizes the efficient utilization of resources.
Minimum Education: Has completed the basic professional curricula of a school of Nursing as approved and verified by a state board of nursing and holds or is entitled to hold a diploma or degree therefrom.
Licensure, Registration and/or Certification: Valid multi-state or State of Oklahoma Registered Nurse License.
Work Experience: Minimum 5 years related experience.
Knowledge, Skills, and Abilities: Knowledge of Microsoft 365 and other applicable software. Working knowledge of general disease state management processes, bio-psychosocial model, and clinical pathways. Excellent communication skills, both written and verbal that present clear and concise information. Effective interpersonal skills. Ability to organize and prioritize work in an effective and efficient manner.
Essential Functions and Responsibilities: Coordinates activities of personnel to guide in driving throughput and decrease Emergency Department (ED) hold times. Arranges, with the assistance of the manager, appropriate staffing levels. Assumes administrative responsibility for Hospitalist Care Facilitators in absence of the manager. Assists to decrease barriers by coordinating workflow with case management, nursing, and social workers. Attends meetings to promote discharge planning. Assists with the clinical portion of the employee evaluations. Serves as a clinical resource for patient care activities and problem resolution. Assists in revision and development of policies, procedures, and established protocols. Ensures smooth communication and acts as a bridge between clinical and administrative roles.
Decision Making: Independent judgment in making decisions involving non-routine problems under general supervision.
Working Relationships: Direct Supervision of others. Has input on performance evaluations but does not prepare or give. Works directly with patients and/or customers. Works with internal and/or external customers via telephone or face to face interaction. Works with other healthcare professionals and staff.
