Community Connections of Moniteau County, Inc. is committed to providing and coordinating support, through person-centered services, to individuals who have intellectual and developmental disabilities. The goal of these services and support is to promote independence, community involvement, and quality of life.

Service coordinators support people with developmental disabilities and their families to identify goals, access needed services, and achieve the highest possible independence and community inclusion. They advocate for individuals and monitor and evaluate services with the person, family, and/or guardian.

· Manage a caseload across ages, disabilities, and complexity by building strong relationships through regular contact and person-centered planning; identify needs and develop action plans with the individual and team.

· Build community partnerships and connect individuals and families to resources identified in the plan; research or develop resources, make referrals, support follow-up, prepare requests for services and service authorizations, and coordinate federal, state, county, and community programs.

· Monitor and assess the quality, timeliness, and effectiveness of services through ongoing review via phone/email and in-person meetings (monthly, quarterly, and annually) in settings such as homes, day programs, community locations, offices, group homes, and residential sites. Follow reporting protocols, including mandated reporting when abuse or neglect is suspected.

· Complete written case notes for all Targeted Case Management (TCM) activities within three business days.

· Maintain current knowledge by completing assigned training, pursuing continuing education, and attending conferences and seminars.

· Support team success by accepting and completing special assignments and duties as needed.

QUALIFICATIONS

REQUIRED EDUCATION

· Bachelor’s degree from an accredited college or university, or a Registered Nurse (RN) license.

REQUIRED MISCELLANEOUS QUALIFICATIONS

· Reliable transportation and acceptable driving record required; must use personal vehicle to travel to individuals’ homes for face-to-face visits. (Mileage reimbursement)

· Valid driver’s license required.

· Proof of valid personal auto insurance required.

PREFERRED EXPERIENCE

· Experience supporting individuals with intellectual and/or developmental disabilities in a community-based setting (e.g., RN or disciplines such as social work, special education, psychology, counseling, vocational rehabilitation, PT/OT, speech therapy, or related fields).

· Experience with case management and documentation systems.

· Experience on multidisciplinary teams assessing, developing, or implementing person-centered plans, especially for individuals with intellectual/developmental disabilities.

· Proficiency with Microsoft Office.

SHIFTS: Full-time (32-40 hours/week), Monday–Friday. No on call required. Hybrid work after 90 days.
