Supportive Housing Case Manager

Job Summary
Care Corps’ LifeHouse is seeking a compassionate and motivated Supportive Housing Case Manager to help individuals and families achieve long-term housing stability and self-sufficiency.

In this role, you will provide direct case management services to participants in our scattered-site supportive housing programs. You will work one-on-one with individuals and families to develop personalized goals, connect them with community resources, and support them in maintaining stable housing.

Responsibilities

  • Meet with participants regularly to develop and implement individualized plans that support housing stability and self-sufficiency.
  • Connect individuals and families to community resources including healthcare, mental health services, employment, education, and public benefits.
  • Provide coaching in life skills such as budgeting, job readiness, communication, and problem-solving.
  • Maintain accurate case notes and documentation in accordance with program standards.
  • Collaborate with shelter staff and community partners to coordinate services and support successful housing outcomes.
  • Monitor participant progress and outcomes, adjusting plans as needed to help individuals maintain stable housing.

Pay: $20.00 - $22.00 per hour
