Supportive Living Services Caregiver

Job Summary
We are seeking a compassionate and dedicated Supportive Living Services Caregiver to join our team, providing vital assistance to individuals in home settings. In this role, you will deliver personalized care that promotes independence, safety, and well-being for residents with diverse needs, including those with developmental disabilities, dementia, Alzheimer's, autism, and other health conditions. Your energetic approach will ensure residents receive attentive support while maintaining their dignity and quality of life. Duties

  • Assist residents with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or resident’s personal assistance devices
  • Monitor residents’ health by taking vital signs, observing patient conditions, and reporting changes promptly to healthcare teams
  • Administer medications following care plans and ensure compliance with HIPAA regulations to protect patient confidentiality
  • Prepare nutritious meals and snacks tailored to individual dietary needs while maintaining cleanliness in kitchen and dining areas
  • Support residents with memory care needs through engaging activities, structured routines, and specialized dementia or Alzheimer’s care techniques
  • Maintain a safe environment by performing cleaning duties, sanitizing common areas, and adhering to infection control protocols
  • Document care activities accurately in resident records, update care plans regularly, and collaborate with social workers and healthcare professionals for comprehensive support

Qualifications

  • Prior experience working in assisted living facilities, nursing homes, group homes or home health environments is highly preferred
  • Knowledge of patient observation techniques, medication administration, vital signs monitoring, and HIPAA compliance
  • Certification in CPR and First Aid is required; additional certifications such as Hoyer lift operation or hospice care are a plus
  • Experience working with individuals with developmental disabilities, autism spectrum disorder (ASD), dementia or Alzheimer’s is desirable
  • Ability to perform heavy lifting safely and assist residents with mobility using appropriate equipment
  • Strong communication skills to effectively interact with residents, families, and healthcare teams
  • Compassionate attitude with a commitment to providing respectful and personalized care
