Targeted Case Manager

We are mental health and addiction recovery clinic looking for a case manager to work 20-25 hours per week between the hours of 8:30am to 1pm. The current case load is up to 25 clients. Our clients come to groups Mon - Friday and our case manager assist in connecting them to the necessary resources. We have an amazingly supportive team, with peer support to assist the clients in accomplishing their treatment goals.

Must have current case management certification and very familiar with community resources.
